苯那普利联合美托洛尔治疗心力衰竭的疗效观察

摘　　要：目的观察苯那普利联合美托洛尔治疗充血性心力衰竭（CHF）的临床疗效。方法200例CHF均经临床表现、超声心动图及心脏二位片确诊，按就诊顺序随机分为A、B两组，A组100例给予苯那普利5—10mg／d，美托洛尔6．25～100mg／d，B组100例服同等剂量安慰剂，两组基础治疗类同。定期来院随访，坚持服药0．5～2．0年，平均服药时间为327d，随访满半年后复查心脏B超，判定疗效的终点事件为临床心功能状况。结果苯那普利联合美托洛尔使CHF患者心率减慢，心功能改善，超声心动图复查显示左室舒张末期内径及左房内径缩小，左室射血分数增高，半年随访心功能改善者明显多于对照组。结论CHF患者长期服用苯那普利加美托洛尔可使其心功能改善，提高生活质量。Objective To evaluate the clinical therapeutic efficacy of banazapril accompanied with metoprolol in treatment of congestive heart failure （CHF）. Methods Two hundred patients with CHF were randomized to take banazapril（5 - 10 mg per day） and metopolol （6.25 - 100 mg per day, n = 50 ）and to placebo （ n = 100 ）on the basis of conventional teratment. The mean periods of follow up was 327 days and the efficacy was assessed by means of eehocardiography and clinical symptoms. Results There were more signficant improvement in NYHA functional class in hanazapril accompanied with metoprolol group than in pal- ceebo group. Banazapril and metoprolol were associated with greater overall reduction of left ventricular end - diastolic diameter and an overall increase in the LVEF% during the first year of therapy. Conclusion Long term addition of banazapril and metoprolol to standard therapy may profoundly improve cardiac function and prevent clinical deterioration in patients with CHF when compared with patients without metoprolol and banazapril treatment.
