老年高血压患者血尿酸与颈动脉粥样硬化关系的临床分析

摘　　要：目的分析老年高血压患者血清尿酸水平和颈动脉粥样硬化程度的关系。方法采用SYSMEX-180全自动生化分析仪以尿酸酶过氧化物酶法测定85例老年高血压患者血清尿酸水平,用超声多普勒检测颈动脉粥样硬化程度。结果颈动脉斑块发生率为62.35%(53/85),其中1级斑块组为21.18%(18/85);2级斑块组为28.24%(24/85),3级斑块组为12.94%(11/85)。血尿酸分别为0级:266.02±58.22umol/L,1级:318.93±48.32umol/L,2级:449.32±51.02umol/L,3级:523.69±47.19u-mol/L,有斑块组血尿酸明显高于无斑块组(P<0.01)。斑块组与无斑块组血尿酸水平的差异有统计学意义(P<0.01)。颈动脉内膜中层厚度正常组、增厚组及斑块形成组3组间血尿酸水平分别为413±59umol/L、492±52umol/L和531±47umol/L,3组间有显著性(P<0.05),随着内膜中层厚度增加,血尿酸水平增高。结论血尿酸是老年高血压患者颈动脉粥样硬化的重要相关因素,血尿酸水平的升高是动脉粥样硬化的危险因素,血尿酸水平的高低可作为老年高血压患者颈动脉粥样硬化病情轻重的评判指标之一。OBJECTIVE To analyze the relationship between the level of Blood Uric Acid（BUA） and Carotid Arterial Scleratheroma in senior hypertension patients. METHODS SYSMEX-180 automatic biochemical analysator was used to detect the level of BUA by uricase peroxidase method and Ultrasonic Doppler Cardio Scope was used to detect the degree of Carotid Arterial Scleratheroma among 85 senior hypertension patients. RESULTS The prevealence rate of plaqque in common carotid artery （CCA） is 62.35% （53/85）, 1 rank： 21.18% （18/85）; 2 rank 28.24% （24/85）, 3 rank 12.94% （11/85）o Uric Acid in blood （umol/L） （0 rank： 266.02±58.22 umol/ L, 1 rank ： 318.93±48.32 umol/L, 2 rank ： 449.32±51.02 umol/L, 3 rank ： 523.69±47.19umol/L）, the level of BUA in the plaque group were significantly increased than that in no plaque group （P〈0.01） .In CCA-intima0media thickness （IMT） in normal, tieken and plaque grorp, BUA （amol/L） were 413±59, 492±52,531±47, there were a significantly difference （P〈0.05）, BUA were inereased by the thicken in CCA-IMT. CONCLUSION BUA is a key factor of carotid arterial seleratheroma in senior hypertension patients and the increase of BUA is a dangerous faetor which can cause earotid arterial seleratheroma. Therefore, the level of BUA is eonsidered to be one of the indexes of the degree of Carotid Arterial Scleratheroma in senior hypertension patients.
