舌下含化硝苯吡啶快速降压作用疗效分析

摘　　要：选择６２例收缩期血压（ＳＢＰ）２６．６７～３７．３ｋＰａ，舒张期血压（ＤＢＰ）１６．０～１８．６７ｋＰａ高血压病患者。立即舌下含化硝苯吡啶２０ｍｇ，观察该药短时间内快速降压效果，结果：３０ｍｉｎ内ＤＢＰ下降２．６７～３．３３ｋＰａ者５０例，占８０．６５％，６０ｍｉｎ内ＤＢＰ下降１．３３～３．６ｋＰａ及ＳＢＰ下降３．３３～１０．９３ｋＰａ者各５８例，占９３．５５％。无效者４例，占６．４５％。在治疗观察中，发现该药对充血性心力衰竭也有一定作用，１２例心衰中经含化该药１～２ｈ后，有９例（７５％）患者临床症状得到明显改善，心率由原来平均１０１次／ｍｉｎ，降至９２次／ｍｉｎ。由于该药具有扩张小动脉降低后负荷，同时也有扩张静脉作用，短时间降压速度快，可口服及舌下含化。因此，该药可作为重症高血压或高血压急症应急性药物，对并发有急性充血性心力衰竭患者也极为有益。In　this　paper,62　cases　hypertensive　patients　were　selected　whose　systolic　pressure　(SP)　is　26.67～37.3Kpa　and　diastolic　pressure　(DP)　is　16.0～18.67　Kpa,then　they　were　given　20mg　Nifedipine　to　observe　the　quick　antihypertension　effect　in　a　short　course.The　result　shows　as　follows.There　were　50　cases　whose　DP　had　declined　2.67～3.33　Kpa　in　30　min　(80.65%)　and　58　cases　whose　DP　had　declinded　1.33～3.6Kpa　and　SP　declined　3.33～10.93　Kpa　in　60　min　(93.55%),only　4　cases　no　effect　(6.45%).The　observer　also　finds　that　nifedipine　has　some　effect　on　congestive　heart　failure. In　that　the　clinical　symptoms　of　9　cases　heart　failure　patient　is　obviously　improved　of　12　cases　patients　after　sublingually　nifedipine　1～2　hours,whose　heart　rate　declines　from　101/min　to　92/min.Generally,the　drug　can　enlarge　arteriole　and　vein　to　decline　the　afterload,So　the　antihypertension　is　quick.On　the　whole,the　drug　not　only　can　act as　emergency　medicine　for　severe　hypertension　and　hypertensive　emergency,but　also　has much　help　for　acute　congestiue　heart　failure..
