摘 要:本文旨在研究严重烧伤病人回吸收时的微循环变化特点。作者采用WX-753B型微循环显微镜和XG-8型图像处理系统,观测了50例年龄18~38岁,烧伤面积31%~91%的严重烧伤病人回吸收时足甲襞微循环(FNM)。对照温为年龄18~36岁的健康成人50例。与对照组比较,烧伤回吸收组FNM管襻清晰度下降(P<0.01),管襻数目减少(P<0.05),输出枝长度增长(P<0.01),输入枝、输出枝和襻顶直径均增大(P<0.01),畸形管襻的数目增多(P<0.01);血流速度减慢(P<0.01),有明显的红细胞聚集(P<0.01),有白微栓形成(P<0.05).乳头下静脉丛(微静脉)增多并扩张充血(P<0.01)管襻周围渗出(P<0.01)等。结果显示,回吸收对严东烧伤病人微循环有明显影响。因此,烧伤回吸收阶段,在进行其它治疗的同时,还应该积极纠正微循环紊乱。This paper studies the characteristics of microcirculatory change of those severely burned patients during the resorption period. The foot nail fold microcirculation (FNM) of the severely burned patientsduring the resorption period was observed by using the microcirculation microscope (WX-753B) with imageanalysis system (XG-8). FNM of 50 cases with the burn area of 31%~91% and at the age of 18~38 years oldwas observed,and 50 normal adults (18~36 years old) were used as control group. In contrast with the control group, FNM of the patients was shown as follows:The reduced clearity of the capillary loops (P<0. 01);the decreased number of the capillary loops (P<0.05 );the lengthened efferent branch length(P<0.01);theenlarged diameter of the afferent, the efferent branches and the top of the loops (P<0.01); the increasednumber of the deformed loops (P<0.01); the slowed b1ood flow rate (P<0.01); the distinct RBC aggregation (P<0. 01); and the white microthrombi form (P<0. 05); the increased number of the subpapillary-layervenous plexus expanded and congested (P<0.01);the more permeability of the loops (P<0.01) etc. The obvious effect on microcirculation of severely burned patients is found during resorption. Therefore,correction ofmcrocirculation disorder must be made in company with treatment of the other problems during resorptionperiod.
