脑梗塞伴发焦虑障碍相关因素研究

摘　　要：目的探索综合医院神经内科病房脑梗塞患者伴发焦虑障碍的相关因素。方法采用多种神经心理量表、卒中量表(NIHSS)以及中风病证候量化诊断标准对105例脑梗塞患者的心理状况、睡眠状况、生活能力、社会支持、神经缺损程度及中医证候要素进行评定,按照SCL-90的焦虑因子分为脑梗塞伴发焦虑障碍组与脑梗塞不伴焦虑障碍组,分析两组差异的相关因素。结果脑梗塞伴发焦虑障碍与性别、神经功能缺损程度、额叶、大脑前动脉供血区、强迫因子、抑郁因子、日常生活能力、社会支持、睡眠状况、"瘀血"证呈正相关,与大脑后动脉供血区呈负相关。结论女性、额叶梗塞、大脑前动脉供血区梗塞、强迫、抑郁、"瘀血"、病情严重,社会支持程度及生活质量差是脑梗塞患者伴发焦虑障碍的危险因素,在防治脑梗塞的基础上应加强心理疏导,以及家庭和社会支持,适当选用抗焦虑药物治疗,促进脑梗塞患者的全面康复。Objective To explore the related factors of cerebral infarction associated with anxiety disorders in General Hospital. Methods We adopted psychological scales, the stroke scale and the quantification and diagnostic criteria of stroke disease syndromes to evaluate psychology, sleep, life skills, social support, nerve impairment and elements of TCM Syndrome of 105 cases of cerebral infarction patients to analyze relevant factors of cerebral infarction associated with anxiety disorders, the patients divided into cerebral infarction associated with anxiety disorders group and cerebral infarction without anxiety disorders group according to the anxiety factor mark of SCL-90. Result Cerebral infarction associated with anxiety disorders was positively correlated with sex, the extent of neurological deficits, the frontal lobe, the anterior cerebral artery feeding areas, obsessive-compulsive disorder, depression, activities of daily living, social support, sleep, ＂Congestion＂ pattern , negatively related with the posterior cerebral artery feeding areas. Conclusion Women, the frontal lobe infarction, the anterior cerebral artery infarction zone, obsessivecompulsive disorder, depression, ＂Congestion＂ pattern, serious condition, poor social support and quality of life are risk factors of cerebral infarction associated with anxiety disorders.
