经腹羊膜腔穿刺输液及羊水置换治疗妊娠晚期羊水过少的临床研究  被引量:1

BClinical Study for Oligohydramnios by Ultrasonic Intervene Amniotic Cavity Fluid Infusion and Amniotic Replacement in the Third Trimester

摘  要:目的:探讨妊娠晚期羊水过少时进行羊膜腔输液和羊水置换的临床意义。方法:118例妊娠晚期羊水过少患者随机分为两组,治疗组经腹羊膜腔穿刺输液,并记录输液量和输液后羊膜腔容积与胎儿情况,对照组给予吸氧、输液等一般宫内复苏方法。结果:治疗组行羊膜腔输液和羊水置换,剖宫产率16.95%,新生儿窒息率15.25%;对照组分别为54.24%、45.75%,两组差异有显著性(P<0.01)。对照组新生儿胎粪吸入5例,新生儿死亡2例,畸形儿4例,而治疗组无1例发生。两组产褥病率差异无显著性(P>0.05)。结论:对妊娠晚期羊水过少患者进行羊膜腔输液或羊水置换,降低了新生儿病率和剖宫产率,是减少围产期母婴病率的有效方法。Objective: To study the value of ultrasonic intervene amniotic cavity fluid infusion and am- niotic replacement with hypamnion in the third trimester. Method: 118 cases with oligohydramnios were di- vided into treatment group(59 cases )and control group (59 cases} randomly. The treatment group was han- dled by amnion infusion through abdomen. The amount of fluid infusion, the capacity of amnion cavity after infusion, and condition of fetal were all taken down . Oxygen inhalation and fluid infusion were performed in the control group . Result: The treatment group successfully received amniotic cavity with ultrasonic inter- vene and amniotic fluid replacement, cesarean rate ( 16.95% ) and neonatal asphyxia rate { 15.25% ) in treatment group were significantly different from control group's cesarean rate 1 54.24% ) and neonatal as- phyxia rate I45.75% ) (P 〈0. O1 ]. 6 cases had meconnium plug syndrome and 2 cases died in control group . 4 cases were with illness and disability, with no cases in treatment group . There was no significantly difference in puerperal rate between two groups { P 〉 0.05 }. Conclusion : The cesarean section rate and neonatal mortality rate could be reduced by treating oligohydramnios with preparturi'ent supersonic intervene amniotic cavity fluid infusion and replacement in the third trimester. It is an effective way in reducing the mortality of mothers and fetuses.
