慢性阻塞性肺疾病和肺心病患者血流动力学及心肌力学的变化

摘　　要：应用力学的理论和方法,对右心室心肌收缩和舒张功能的有关指标进行探讨,指标包括右心室±dp/dt、±Vmax、±VCE10、T值。分析52例慢性阻塞性肺疾病(COPD)和肺心病患者血流动力学和心肌力学变化。反映心肌舒张性能的指标Vmax、VCE10两组间有显著差异(P<0.01或P<0.05)。Vmax是一个既能定量反映心肌收缩力,而又不受前后负荷影响的心肌力学指标。其余指标除—Vmax外,±dt/dt、—VCE10、T值均受心室前后负荷的影响。Using the theories and methods of mechanics to approach the methods of measurement and relative indices of the diastolic and systolic functions of right ventricular myocardium, e g. indices of my-ocardial mechanics of right ventricular ±dp/dt, ± Vmax, ± VCE10,T value variation of myocardial mechanics in 52 patients with chronic obstructive pulmonary disease (COPD) and cor pulmonale were analyzed. The indices Vmax,VCE10,which showed the function of myocardial diastole,had remarkable differences between the two groups (P<0. 01, P<0. 05); The Vmax is an index of myocardial mechanics , which can show the contractility of myocardium, and it is not affected by preload and post-load. The other indicesCnot include -Vmax) ,e g. ± dp/dt,±VCE10.T value,are affected by preload and post-load of the ventricle.
