摘  要:探讨血清神经元特异性烯酸酶(NSE)、血浆内皮素-l(ET-1)与新生儿缺氧缺血性脑病(HIE)预后的关系。方法分别用酶联免疫法和放射免疫法动态测定25例正常足月新生儿和59例足月HIE新生儿血清NSE浓度和血浆ET-1浓度(生后24小时内、第3天、第7天)。并于生后3个月、6个月用Gesell法进行神经发育检查。结果血清NSE、血浆ET-1浓度与新生儿HIE程度呈正相关;生后3天内NSE、ET-1浓度显著增高及第7天浓度持续不降者预后不良。结论生后第3天血清NSE与血浆ET-1浓度可早期预测新生儿HIE近期预后,NSE预测预后的特异性高于ET1,动态观察则预后价值更高,对临床早期干预以减少中枢神经系统后遗症的发生有重要指导意义。To investipate the relationship between nearon speific enolase (NSE) concentraition in, endothelin- 1 (ET- 1 ) in plasma andthe neurologic prognosis of full term neonals Hypoxic-ischemic encephaloptky(HIE).Methods The serum NSE concentrations of 59 HIE infants (20 cases oflight, 20 of middle and 19 of severe degree) and 25 normal infants were measured by enzyme immunoassay dynamically (within 24 hours, at the 3rd dayand the 7th day of life); ET- 1 in Plasma were measured by radioimmunoassay. Neurological development examinations(Gesell) were done when the patients were 3 and 6 months old.ResultS NSE and ET-1 within 3 days of life were related to the degreeof HIE; At the 7th day, NSE and ET-1 concentrations decreased rapidly butthe milddle and severe degree's were higher than the light and normal' s.Those who had higher NSE and ET-1 concentrations within 3 days Of life andwhose concentrations did not decrease at the 7th day had advent prognosis.Conclusion The results indicate that NSE and or- 1 are reliablebiochemical markers for any estimable of hypoxic-isckemic brain damage andprognosis in am full HIE infants, and the NSE is more superior to ET-1.
