自动痔疮套扎术(RPH)治疗轻中度痔疮疗效观察(附153例报告)

摘　　要：目的研究痔疮自动套扎术(RPH)治疗轻中度痔疮的疗效。方法将278例轻中度(Ⅰ～Ⅲ)痔患者按治疗方式不同分为两组:RPH组行RPH治疗153例,对照组行微波治疗125例。其中RPH组中以出血及单点脱垂(脱出痔核≤2个)症状为主的病人只行1次套扎术,环形脱垂患者(脱出痔核>2个)相隔4周后行第2次套扎术;微波组环形脱垂患者行分期分部位2次治疗,时间相隔4周,每次治疗痔核不超过4个。结果RPH组治愈146例(95.4%),微波组治愈108例(86.4%)。术后随访半年,RPH组术后回缩不全4例(2.6%),3例复发(1.9%),微波组12例回缩不全(9.6%),5例复发(4.0%)。术后调查RPH组150例对手术效果表示满意(98.0%),微波组114例表示满意(91.2%)。RPH组患者术后无肛管疤痕形成、肛门狭窄、出血等并发症。结论RPH作为治疗轻中度痔的一种方法,不但具有极佳的疗效,而且操作简单,费用低,不用住院及麻醉,患者痛苦轻微,几无并发症,可重复操作,值得广泛推广。Objective To study the efficacy of automatic ligation of hemorrhoids （RPH） in the treatment of mild and moderate hemorrhoids. Methods A total of 278 patients were divided into two groups： 159 patients in RPH group and 125 patients in microwave therapy group. The patients in RPH group with the main symptom of bleeding and single prolapsed hemorrhoid（≤2） only needed RPH one time while circle prolapsed hemorrhoid （〉2） patients needed RPH the second time 4 weeks later. The patients with circle prolapsed hemorrhoid in the microwave therapy group needed the treatment two times. Each time for different part of the hemorrhoid. Results The symptoms of all the patients were improved in different degree, which were completely eliminated in 146 patients in RPH group （95.4%） while in 108 patients in micro-wave therapy group（86.40%）. According to an observation half year after operation, the prolapsed hemorrhoids in RPH group of 4 cases （2.6%） were not completely retracted and 3 patients relapsed （1.9%） while there were 12 cases of incomplete retraction（9.6%） and 5 cases of relapse（4.0%） in the microwave therapy group. Questionnaire of patients＇ satisfaction shows that 150 cases （98%） in RPH group were satisfactory while 114 cases in microwave therapy group felt satisfactory （91.2%）. There were no complications of scare formation in anus, anal stenosis and bleeding in RPH group. Conclusion RPH is an excellent method for mild and moderate hemorrhoid. It is not only possessing an excellent efficacy, easily manipulated, but also unnecessary with anesthesia and hospitalization and has no complication. This method is worth to be popularized.
