摘 要:目的:研究葡萄糖对35%TBSAⅢ°烫伤大鼠肠内补液时小肠电解质溶液吸收率的影响。方法:雄性Wistar大鼠30只,随机分为:假烫补林格氏液组(假烫LR组,n=10);烫伤补林格氏液组(烫伤LR组,n=10)和烫伤补葡萄糖-林格氏液组(烫伤LR/G组,林格氏液中加入20g/L的葡萄糖粉,n=10)。烫伤组采用沸水法(100℃,15 s)造成35%TBSAⅢ°烫伤。各组于烫伤后0.5h按1/2派克兰公式量和速率(2mL/kg.1%TBSA-1)进行肠内补液。测定烫伤后4h内肠道对水分和Na+的吸收率,以及伤后4h肠黏膜血流量和Na+-K+-ATP酶活性。结果:烫伤LR组小肠对水和Na+的吸收率明显下降(P<0.05);两烫伤组之间比较,烫伤LR/G组对水和Na+的吸收优于烫伤LR组,在伤后2h有显著差别(P<0.05);伤后4h烫伤LR/G组肠黏膜血流量和Na+-K+-ATP酶活性也显著高于烫伤LR组(P<0.05)。结论:35%TBSAⅢ°烫伤大鼠肠内补液时,小肠对葡萄糖电解质溶液的吸收率明显下降;葡萄糖可以提高烫伤休克肠内复苏时小肠的吸收率,并且与提高肠黏膜血流量和Na+-K+-ATP酶活性有关。Objective :To investigate the effects of glucose on electrolyte absorbance during enteral resuscitation in scald rats, and evaluate the effects of glucose on the enteral absorption of electrolyte and water. Methods:Male Wistar rats (n = 30) were randomly divided into three groups:sham scald with enteral Ringer's solution( sham LR, n = 10), scald with enteral LR( scald LR, n = 10 ), and scald with enteral LR containing 20g/L glucose group (scald LR/G, n = 10). About 35% TBSA of scald rats were produced with boiling water( 100℃ ,15s). The amount and speed of infusion of resuscitation fluid were estimated according to 1/2 of Parkland formula(2mL/kg·1% TBSA^-1) within 0. 5h after injury. During the first 4 hours after scald, enteral absorption rate of water and Na^+ were continuously assessed. Intestinal mucosa blood flow and the activity of Na^+ -K^+ -ATPase was determined. Results: The enteral absorbance of water and Na^+ in scald LR were significantly decreases (P〈0. 05) ; The intestinal absorption rate of water and Na^+ in scald LR/G group were significantly enhanced compared to those in scald LR group from 2h after injury (P〈0.05) ; There was also much significant higher of intestinal mucosa blood flow and activity of Na^+ -K^+ -ATPase in scald LR/G group than that in scald LR group at 4h after scald. Conclusion:The results indicated that 35% TBSA of scald injury can lead to lowering the gut absorption rate of water and Na^+ in rats. Enteral resuscitation with the glucose-electrolyte solution is superior to electrolyte solution only in its improvement of intestinal absorption and the activity of Na^+ -K^+ -ATPase.
