摘  要:目的探讨超声造影(CEUS)技术诊断前列腺癌的准确率。方法对临床拟诊26例前列腺癌患者(共34个结节)进行CEUS检查,观察病灶造影后增强强度,并与对侧或周围正常组织对比,依据增强强度不同分为高增强、等增强、低增强和无增强四种不种的增强方式,结合时间强度曲线(TIC)判断病灶性质。结果经穿刺活检病理诊断30个结节为前列腺癌,4个为前列腺增生结节。癌结节中有25个呈高增强,且强度明显高于正常组织,而所有增生结节与正常组织几乎同时增强,强度与正常组织相当。结论CEUS能较为客观正确地评价前列腺内病灶的血流情况,可以引导进行穿刺活检;TIC可定量分析病灶的增强强度及方式,有助于前列腺癌的早期诊断。Objective To evaluate the accurate rate of contrast enhanced ultrasound(CEUS) in the diagnosis of prostate cancer. Methods Twenty-six patients(34 nodules) with the diagnosis of prostate cancer were examined with CEUS. Flow signals within the lesions were classified as increased, equal, decreased and no flow compared within the surrounding normal tissues. The results were analyzed with time-intensity curves (TIC), by which prostate cancer was differentiated from benign lesions. Results Thirty nodules were pathologically diagnosed to be prostate cancer and 4 hyperplastic nodules by biopsy. Flow signals of 25 cancer lesions increased and the intensity was higher than those of normal tissues. However, time to enhancement and peak intensity of hyperplastic nodules was about equal to that of normal tissues. Conclusion CEUS can he used to evaluate the presence of vasculature within the lesions of prostate cancer and to guide the prostate biopsy. TIC may be helpful in quantitively analyzing the intensity of the lesions and in early diagnosing prostate cancer.
