摘 要:目的研究131I标记的肿瘤细胞核人鼠嵌合单克隆抗体(131I-chTNT)在分化型甲状腺癌肺转移病人体内的生物学分布。方法10例分化型甲状腺癌患者,单次静脉注射131I-chTNT(5MBq/kg体重)后测量各时间点血、尿样放射性;应用超饱和三氯醋酸检测131I-chTNT在体内的稳定性和代谢情况。采用连续显像法勾画感兴趣区估算全身、肿瘤和主要器官不同时间点的摄入百分比。结果静脉推注131I-chTNT后血清时间放射性曲线结果符合单次静脉给药二室模型,T1/2(65.28±14.83)h,AUC0-t(8.93±1.32)MBq.h-1.ml-1,AUC0-∞(10.58±2.19)MBq.h-1.ml-1,CL(1635±359.15)ml.min-1.kg-1;静脉推注131I-chTNT后尿液时间排除放射性百分比曲线符合单次静脉给药一室模型,T1/2(99±9.6)h,1w累计有注入剂量(31±9)%的放射性经尿液排泄;血清中131I-chTNT占放射性成分的百分率,其中24,48,72h血清中的131I-chTNT均超过95%,168h131I-chTNT仍达到(88±7)%;尿中放射性物质的化学成分,24,48,72,168h尿液样本放射性100%源于游离的131I;131I-chTNT静脉推注后放射性主要浓聚在肺和肝,脑最少。肿瘤组织放射性24h达到最大值,T/N比值在3~7d达到最大值(1.28~3.83)。结论131I-chTNT在分化型甲状腺癌肺转移病人体内的生物学分布特点有可能为该肿瘤的治疗提供帮助。Objective To study in vivo biological distribution of the iodine-131-labeled mouse/human chimera monoclonal antibody (^131 I-chTNT) in patients with pulmonary metastases from differentiated thyroid carcinoma. Methods 10 patients with differentiated thyroid carcinoma were injected intravenously with a single dose of ^131 I-chTNT (5 MBq/kg body weight), and then radioactivity of blood and urine samples were measured at different time points. In vivo stability and the metabolic status of ^131 I-chTNT were detected with supersaturated trichloroacetic acid. Continuous imaging was performed to outline the region of interest (ROI) and estimate the percentage of intake for the whole body, major organs and tumor lesions at different time points. Results The result for the serum time-radioactivity curve of ^131 I-chTNT accorded with the two-compartment model after a single intravenous injection, T1/2(h) 65.28±14. 83, AUCO - t ( MBq·h/ml) 8.93±1.32, AUCO-∞ (MBq·h/ml) 10. 58±2. 19 and CL (ml·min^-1 ~ kg^-1) 1 635±359. 15. The result for the urine excretion time-radioactivity percentage curve of ^131 I-chTNT accorded with the one-compartment model after a single intravenous injection, T1/2 (h) 99±9. 6, and accumulative (31±9) % radioactivity of the injected dose was excreted within urine in one week. The percentages of serum ^131 I-chTNT in radioactive components at 24, 48 and 72 h were all above 95% and that at 168 h, still reached (88±7) %, as for chemical composition of radioactive substances in urine, 100% radioactivity in urine samples originated from free ^131 I. Radioactivity of ^131 I-chTNT after intravenous administration was mainly concentrated in the lung and liver, least in the brain. Radioactivity of tumor tissues reached the maximum at 24 h and the tumor/normal tissue (T/N) ratio reached the maximum (1.28±3.83 ) during 3 -7 d.
