实时三维超声心动图显示肌部室间隔缺损心室面形态的效果  

Display Effect on Ventricular Surface Shape of Muscular Ventricular Septal Defect Using Real-time Three-dimensional Echocardiography

摘　　要：目的评价实时三维超声心动图（RT3DE）对肌部室间隔缺损（VSDm）心室面形态的显示效果及其影响因素。方法31例VSDm患者接受了RT3DE，其中23例经手术证实，其余8例经CT／MRI检查证实。VSDm位于房室瓣瓣环附近2例，位于室间隔中部6例，位于室间隔中部及心尖部5例，位于室间隔中部与游离壁交界处6例，位于心尖部11例，位于心尖部与游离壁交界处1例。应用飞利浦7500实时三维超声成像仪对VSDm进行多角度三维灰阶成像。确定RT3DE显示VSDm心室面形态的效果并评价VSDm位置、大小、走行、右室面肌束、图像质量和参考切面等因素是否对其有影响。结果RT3DE可显示54．8％（17／31）VSDm的心室面形态，45．2％（14／31）VSDm心室面形态则显示不清。VSDm位置（P〈0．001）和走行（P〈0．01）对RT3DE显示VSDm心室面形态有显著影响，VSDm大小也有一定的影响（P〈0．05）。结论RT3DE可显示半数以上VSDm心室面形态，其效果受VSDm大小、位置和走行等多种因素的影响。Objective To evaluate the display effect on the ventricular surface shape of muscular ventricular septal defect （VSDm）by real-time three dimensional echocardiography（RT3DE）and its affecting factors. Methods A to tal of 31 patients with VSDm, among whom 23 were confirmed by surgical operation and the remaining 8 by CT or MRI,underwent RT3DE. VSDm was found on the vicinity of atrioventricular ring in 2 cases;in the middle part of yen tricular septum in 6 cases;in the middle and apical parts of ventricular septum in 5 cases;on the commissure of the free ventricular wall and the middle ventricular septum in 6 cases;in the apical part of ventricular septum in 11 cases and on the commissure of the ventricular free walls and the apical ventricular septum in 1 case. Multi-angle three-dimensional imaging was performed in VSDm with Phillips Sonos 7500 RT3DE machine for observing the display effect of the ventricular surface shape in VSDm on RT3DE and for determining whether it was affected by the position,size and course of VSDm,muscle bundle of right ventricle, image quality, and referenced cross-section, etc. Results The ventricular surface shape of VSDm was clear on RT3DE in 54.8%（17/31）of the cases,and unclear in 45.2%（14/31）. The position（P〈0. 001）and course（P〈0.01） VSDm had a significant impact on the display of its ventrieular surface shape on RT3DE,while the size of VSDm also had some effect （P〈0.05）. Conclusions RT3DE may display the ventricular surface shape of VSDm in over half of the patients,hut the effect may he affected by the size, position and course of VSDm.
