无透视经胸超声心动图引导房间隔缺损封堵术的应用

摘　　要：目的方法结果呈强回声，结论探讨无透视经胸超声心动图引导房间隔缺损封堵术的方法和可行性。10例房间隔缺损患者，全程经超声引导进行封堵，包括引导导管的走行，指导封堵器的释放，监测手术的安全性。10例手术顺利完成，成功率100％，手术平均时间52min，无1例发生心包积液及心内结构损伤。超声监视下导丝外鞘管为2条强回声呈“等号状”，导管顶端位置可注生理盐水明确。房间隔缺损患者单纯经胸超声心动图引导封堵术是可行的，可免除X线辐射，有较好的应用前景。Objective To discuss the method and feasibility of transthoracic eehocardiography in closure of atrial septal defect without x-ray. Methods 10 atrial septal defect patients were received operation of closure of atrial septal defect. The whole process were guiden by transthoracic echocardiography completely including catheter＇s direction, occluder releasing, and monitoring safety of the whole opration. Results All operations were successful, and the success rate was 100%, mean time of the operation was 52 min, no one occurred pericardial effusion and other damage of structure of heart. The guide wire appeared hyperechoic, sheath appeared hyperechoic parallel line as equal-sign, and the end of catheter may determined by injecting normal saline. Conclusions the method of transthoracic echocardio-graphy in closure of atrial septal defect without x-ray is feasible, it can avoid the radiation of x-ray, and has a good prospect.
