摘 要:目的探讨应用三维超声诊断胎儿小脑蚓部发育不全的价值。方法经二维超声小脑横切面检查疑有先天性蚓部发育不全的9例胎儿,进一步应用三维扩展成像多切面同屏显像技术和三维矢状切面重建技术检测胎儿小脑蚓部的形态和大小,以及蚓部与第四脑室、脑干的关系。结果2例由于扫描切面为半冠状切面误诊为蚓部发育不全的正常蚓部;3例诊断为蚓部发育不全;2例诊断为蚓部轻度上旋而蚓部发育正常;2例诊断为后颅窝积液。这些病例经产前、产后的超声或MRI,或尸检证实三维超声诊断结果。结论二维超声小脑横切面检查怀疑小脑蚓部发育不全时,应当进一步应用三维超声进行细致的扫查,在重建的蚓部矢状面重新加以评估,注意鉴别小脑蚓部发育不全和蚓部上旋。Objective To discuss the value of 3D ultrasound for diagnosis of fetal cerebellar vermis agenesis. Methods Totally 9 fetus who were suspected agenesis of the vermis on axial plane, underwent further evaluation of the vermis morphous and size, the relationship between the brain stem and the vermis and fourth ventricle using the multi slice view of three-dimensional extended imaging 3DXI and the 3D sagittal plane reconstructed. Results Vermis agenesis were misdiagnosed because of the semi-coronal section in two cases; vermis agenesis were found in three cases; the vermis size within normal limits for gestational age with mild posterosuperior rotation were found in two cases; enlarged cisterna magna were found with cerebellar vermis mild rotation in two cases. Findings were confirmed by follow-up. Conclusions When vermis agenesis is suspected in standard axial plane by 2D ultrasound, further evaluation on the vermis should be conducted by three-dimensional ultrasound, it is in the sagittal plane reconstructed to identify cerebellar vermis agenesis or vermis rotation.
