摘  要:目的:对皮下埋植避孕剂的副反应、月经异常停用率和可接受性进行评价。方法:本研究为多中心回顾性对比研究,采用多元COX回归逐步法进行多因素分析。结果:研究对象在使用皮埋剂2年内月经紊乱发生率最高,其中点滴出血/经期延长,占40%。续用皮埋剂者在2年内恢复月经正常者较停用者多,两者比较有显著性差异。体重轻、末次避孕方法因副反应停用、使用皮埋剂后影响性生活、体重变化大、出现点滴出血/经期延长者因月经异常而停用皮埋剂的风险率较高。年龄轻、人流次数多者对皮埋剂的耐受性较好。结论:在使用皮埋剂的前2年,积极治疗皮埋剂引起的月经紊乱,可以提高皮埋剂的续用率和接受性。Objective: To evaluate the contraceptive efficacy and menstruation disturbances of levonorgestrel subdermal implants. Methods: A retrospective comparative clinical study was done on the women accepted levonorgestrel subdermal implants, and the results of stepwise COX regression showed that women were more likely to discontinue implant using due to menstrual disturbances. Results: A multicentre study was analyzed to study the factors that may be associated with discontinuation of levonorgestrel subdermal implants due to menstrual disturbances. The commonest side effect was menstrual disturbance, and the spotting or prolonged bleeding rate for menstrual disturbance was 40%. Menstrual disturbance was the predominant reason for discontinuation of implant using. After two years' treatment, 51. 5% of continued user had normal menstrual. Women were more likely to discontinue implant using due to menstrual disturbances if they were older, less in body weight, less abortion, had experiences of discontinued former contraception use because of side effect, had longer contraception time, and had an effect on sexual desire because of menstrual disturbances. Conclusion: During two-year follow-up, it could increase the compliance rate of levonorgestrel subdermal implants by control the menstrual disturbances.
