摘　　要：目的探讨手术医疗临床路径管理对子宫肌瘤医疗费用控制的效能和可行性,为医疗付费方式改进提供循证医学证据。方法以子宫肌瘤手术医疗临床路径为基础,以手术类型为基点设计相应临床路径,对不同临床路径控制下的同类手术治疗医疗费用情况和相同临床路径控制下的不同类手术治疗医疗费用情况进行对照研究,通过医院HIS系统提取相关数据并建立数据库,并对各组数据进行相关的统计学分析。结果不同临床路径控制下的同类手术治疗产生的医疗费用不同、各种医疗费用构成不同、变异也不同,相同临床路径控制下的不同类手术治疗产生的医疗费用不同、各种医疗费用构成不同、变异也不同。结论子宫肌瘤手术医疗临床路径对医疗费用控制效果良好,为其他病种医疗费用控制提供了新思路,也为建立一种全新的付费机制和临床路径的设计提供了新的思路。Objective To study the efficacy and feasibility about the costs of operation＇s administration controlled by clinical pathway of hysteromyoma. Providing evidence to improve the control of medical costs based on clinical pathway of hysteromyoma. Method Base on study of clinical pathways on myoma of uterus. Method Establishing the corresponding clinical pathways based on different operation types, and comparing the medical costs of different operation types under different clinical pathways. Setting up database through HIS and statistically analyzing the related data. Result The different medical operations under different pathways costed differently. The difference included the component and the variation of the cost. Conclusion The effect of clinical pathways on hysteromyoma operations controlled by the medical costs is very helpful, which providing new ideas on controlling the medical costs of other diseases, building a kind of new payment mechanism and the design of clinical pathways.
