口服钆喷酸葡胺溶液行磁共振泌尿系水成像的临床应用  被引量:2

摘  要:目的探讨口服钆喷酸葡胺(Gd-DTPA)溶液作为胃肠道阴性对比剂降低胃肠道高信号背景,提高磁共振泌尿系水成像(MRU)图像质量的方法。方法42例泌尿系疾病患者采用德国Siemens公司Symphony超导1.5 T型磁共振扫描仪进行检查。在完成泌尿系常规横断面、冠状面扫描后行MRU扫描。MRU采用半傅立叶单次激发快速自旋回波序列(HASTE),于口服300 mL Gd-DTPA溶液前后分别进行MRU检查,并比较其检查效果。肾盂、输尿管及膀胱走行区无胃肠道液体高信号干扰为MRU图像质量良好;MRU图像中如果能够明确病变部位、形态、大小者为能够判定病变情况对疾病作出影像诊断。结果MRU扫描显示口服Gd-DTPA溶液后胃肠道内液体高信号均较口服前有明显下降,口服Gd-DTPA溶液前肾盂、输尿管及膀胱显示良好例数分别为25例、30例、31例,口服Gd-DTPA溶液后肾盂、输尿管及膀胱显示良好例数分别为40例、39例、37例,使MUR图像质量显著提高(P<0.01)。口服Gd-DTPA溶液后新增明确判断病情状况5例,比口服前诊断准确率提高了11.9%。口服Gd-DTPA溶液前胃肠道准备时间(6.2±1.3)h,口服Gd-DTPA溶液后胃肠道准备时间仅需(1.1±0.2)h,使MRU胃肠道准备时间明显缩短(P<0.01)。结论口服Gd-DTPA溶液能安全有效地降低胃肠道液体高信号,改善MRU图像质量。Objective To investigate the method of using gadopentetic acid (Gd-DTPA)as an oral negative gastrointestinal contrast agent to reduce hyperintensities of gastrointestinal tract and improve the image quality of magnetic resonance urography (MRU). Methods Forty-two cases were performed with MRU before and after using 300mL diluted Gd-DTPA which was used as an oral negative contrast agent. All MR images were acquired using a 1.5T MR scanner by HASTE ( half-fourier acquisition single-shot fast spin echo) sequence. The MRU image quality was compared before and after using oral contrast agent. The high quality of MRU image was that there was not interference with the urology system by the high signal intensity from the stomach and intestinal fluid. It was diagnosis the disease if that the location, shape and size of the lesion was founded. Results In all patients, the high signal intensity from the stomach and intestinal fluid was suppressed satisfactorily. The good depiction cases of renal pelvis, ureter and bladder before using oral contrast agents were twenty-five, thirty and thirty-one respectively, and the cases of good depiction were forty, thirty-nine and thirty-seven after using the contrast agents respectively. The image quality was markedly improved by the oral contrast agent( P 〈 0.01 ). There were additional five cases whose lesions were diagnosed after using oral contrast agent. The diagnostic accurate ratio of MRU improved about 11.9% comparing to that before using contrast agent. Meanwhile, the preparation time was (6.2 ± 1.3 ) h and ( 1.1±0.2 ) h before and after using contrast respectively. The preparation time was shortened significantly ( P 〈 0.01 ). Conclusion Oral Gd-DTPA can be used as an effective and safe negative contrast agent in eliminating signal intensity of the gastrointestinal tract, and it can improve the quality of MRU.
