超声心动图在多发性房间隔缺损封堵中的应用价值

摘　　要：目的:探讨经胸及经食管超声心动图在多发性房间隔缺损(ASD)封堵中的应用价值。方法:9例经超声心动图诊断为2处ASD的患者(7例行经食管超声心动图检查)在经胸超声心动图监测下行经心导管房间隔缺损封堵术。结果:7例行经食管超声心动图检查者,5例明确为2处ASD,1例为单个较大缺损,1例为3处缺损。余2例介入封堵术中造影检查示1例为2处缺损,1例为单个缺损。所有患者均在经胸超声心动图监测下行经心导管ASD封堵获得成功。4例患者用了2个封堵器,5例用了1个封堵器。结论:对于多发性ASD,可应用经胸或经食管超声心动图明确缺损的数量、大小、间距以及边缘距离,然后根据缺损间距选择单个或多个封堵器。Objective：To assess the value of echocardiography in transcatheter closure of multiple atrial septal defects （ASD）. Methods：Nine patients with double ASDs diagnosed by transthoracie echocardiography （TTE） ESeven of them underwent transesophageal echocardiography（TEE） ] . All the patients underwent transcatheter closure of ASD under the guidance of TTE. Results：Among the seven patients who underwent TEE, 5 patients had two ASDs, 1 patient had one single big defect, the other had three ASD. The results of angiography of left two patients showed one patient had two ASDs, the other had one single defect. All the patients had succeseful operations. Double occluders were implanted in 4 of 9 patients, while the other 5 patients used one single oeeluder. Conclusion： Eehocardiograhy （TEE and TTE） is valuable for dignos is of ASD, measure the size of the defect, the distance between the defects and the size of the margin. The distance between the defects is very helpful for select single or multiple occluders.
