摘  要:目的:观察促红细胞生成素(EPO)对大鼠急性心肌梗死后的心肌保护作用。方法:采用结扎冠状动脉前降支的方法建立大鼠心肌梗死模型。治疗组大鼠(n=20)采用腹腔注射方法给予EPO(3000U/kg),共3d(术前1d、当天、后1d),对照组大鼠(n=20)相同的时间点给予等量的生理盐水。术后7d各组随机选取5只大鼠,尾静脉抽血,采用ELISA法检测血清肿瘤坏死因子-α(TNF-α)、白介素-6(IL-6)的含量。术后28d后经右颈总动脉插管检测血流动力学指标,心肌组织Ⅷ因子免疫组织化学染色和凋亡蛋白Bcl-2、Bax免疫组化染色。结果:相比对照组,治疗组大鼠血清TNF-α、IL-6水平降低(P<0.05);EPO干预后,心肌组织Bcl-2蛋白表达上调,Bax表达下调(P<0.05),Bcl-2/Bax值显著增高;梗死周围区毛细血管密度明显增加(4.3±0.7)个/视野,(3.6±0.9)个/视野,P<0.05),血流动力学指标左室收缩压、左室压力最大上升速率明显升高,而左室舒张末压、左室压力最大下降速率明显降低(P<0.05)。结论:EPO通过促进毛细血管形成、抑制梗死周围区炎症反应及减少心肌细胞凋亡而达到改善急性心肌梗死后心脏功能的作用。Objective To investigate the protective effects of erythropoietin (EPO) on heart in rats after acute myocardial infarction (AMI). Methods Rat models of AMI which were built by ligating the left anterior descending coronary artery were divided into two groups, treatment group (n = 20) received 3 000 μ/kg EPO through introperitoneal injection for 3 days (1 day before operation, the operation day, and 1 day after operation), and control group (n = 20) received the same volume of saline through the same procedures. Serum TNF-α and IL-6 in 5 rats in each group were measured by ELISA 7 days 'after operation. The hemodynamics were tested through arterial cannula in the right common carotid artery, the Ⅷ factor and Bel-2, Bax of apoptosis protein were stained by immunohistochemistry. Results As compared with control group, the level of TNF-α and IL-6 in treatment group were obviously decreased. After treated with EPO, the expression of Bcl-2 was increased, while Bax was decreased, and the Bcl-2/Bax was increased (P 〈 0.05). The capillary density in the peripheral zone of myocardial infarction was obviously increased (4.3 ± 0.7,3.6 ± 0.9 ,P 〈 0.05). LVSP and+dp/dtmax were increased while the LVEDP and -dp/dtmax were decreased (P 〈 0.05). .Conclusion EPO can improve the cardiac function after AMI in rats through the pro-angiogenic, anti-inflammatory, and anti-apoptotic effects.
