糖尿病视网膜病变相关因素分析

摘　　要：目的了解糖尿病视网膜病变（DR）的相关因素。方法测定92例代谢综合征（MS）、80例2型糖尿病（DM）患者血糖、糖化血红蛋白（HbA1c）、血脂、C反应蛋白（CRP）、血压、身高、体重，计算体重指数（BMI）。应用免散瞳眼底拍照行眼底筛查。分析DR与年龄、糖尿病及高血压病程、血糖、HbA1c、血脂、BMI的相关性。结果MS组DR发生率高于DM组（P〈0．05）。DR组CRP、餐后2小时血塘、HbA1c、甘油三脂、血压、MS患病率高于非DR组（P均〈0．05），糖尿病和高血压病程长于非DR组（P均〈0．05）。DR与糖尿病病程和CRP浓度正相关（β=0．250、0．354，P=0．000、0．007）。结论DR与长期高血糖、高血压、脂代谢紊乱及CRP密切相关。Objective To study the associated factors affecting development of diabetic retinopathy （DR）. MethodsEye ground was determined in 172 patients by non-mydriatic fundus camera screening. These patients were .：livided into two groups：92 patients with metabolic syndrome （MS group） and 80 patients with type 2 diabetes mellitus （DM group）. Fasting and post-prandial 2 h plasma glucose, HbA1 c, fasting plasma lipids, C-reactive protein （CRP） were compared between these two groups of patients. Body height and weight were also measured. BMI was calculated. The relationship of these factors with DR was analyzed. Results The prevalence of DR in MS group was higher than that in DM group（P 〈0.05）. The levels of CRP,2hPG,HbAlc,TG,blood pressure in patients with DR were higher than those without DR（P 〈 O. 05 ）. The prevalence of MS was also higher in patients with DR than those without DR. （ P 〈 0. 05 ）. The duration of DM and hypertension was also higher in patients with DR than those without DR （ P 〈 0. 05 ）. Multiple linear regr~,ssion showed that the development of DR was positively correlated with the duration of DM （β=0.250,P ： 0.000）. and the level of CRP（β =0.354,P =0.007）. Conclusion The development of DR is significant/y correlated with long duration of DM and hypertension. It＇ s also significantly correlated with hyperlipemia and high level of CRP.
