胸部手术后乳糜胸的治疗

摘　　要：目的探讨胸部手术后乳糜胸的治疗方法，总结治疗经验。方法回顾性分析15例胸部手术后乳糜胸的临床表现，手术治疗方法与转归。13例采用右剖胸结扎胸导管断端远侧（近膈端），1例采用胸内注射红霉素溶液，1例采用胸内注射顺铂溶液。结果14例治愈。1例死于恶液质。结论胸部手术后乳糜胸可采用保守治疗或手术治疗，结扎胸导管断端远侧（近膈端）是手术治疗乳糜胸较好的方法。Objective To explore the therapeutic methods of postoperative chylothorax and summarize experience of chlothorax treatment. Methods Fifteen cases of chylothorax postoperation were analyzed retrospectively about their clinical features, operative methods and clinical effects, including 13 cases receiving re - operation by ligating thoracic duct at distant end of the broken site, 1 case receiving intrapleural administration of erythromycine,and 1 case receiving cisplatin. Results Among 15 cases, 14 were cured and 1 died from exhaustion. Conclusion The postoperative chylothorax can be cured by drug or operation. It is an only effective therapy for operation to ligate the thoracic ducts at distant end of the broken ends.
