摘 要:目的研究骨性关节炎(OA)软骨组织中细胞因子-胰岛素样生长因子Ⅱ(IGF-Ⅱ)的基因表达的变化规律。方法选取OA患者(6例)和创伤性截肢的患者(5例)的膝关节软骨组织,HE染色光镜观察OA软骨组织病理学特征。采用原位杂交方法定位软骨组织中IGF-ⅡmRNA的表达。提取关节软骨组织的总RNA和蛋白质,RT-PCR和Western印迹杂交方法测定软骨组织中IGF-ⅡmRNA和蛋白质的表达。结果OA软骨组织表面粗糙不平,软骨细胞排列紊乱,数目减少,有细胞成簇现象,可见软骨细胞变性、坏死,软骨厚度变薄,基质染色不均匀,有异染性。正常软骨细胞的细胞核与细胞浆中有IGF-ⅡmRNA的表达。与正常成人相比,OA患者软骨组织内IGF-Ⅱ在基因的转录与翻译水平明显降低了75%和63%(P<0.05)。结论细胞因子IGF-Ⅱ在OA软骨细胞中呈低表达状态。IGF-Ⅱ基因的差异表达可能是骨关节软骨退变的发生机制之一。Objective To study the genetic expression of insulin-like growth factor Ⅱ ( IGF- Ⅱ ) in cartilaginous tissue in osteoarthritis (OA). Methods Cartilaginous tissues of knee joint were obtained from 6 patients with OA and 5 patients with traumatic amputation. Characteristic pathology of cartilaginous tissue was observed with HE straining. The location of IGF- Ⅱ mRNA in chondrocyte was identified by in situ hybridization. The expression of IGF- Ⅱ mRNA and protein in cartilaginous tissue were determined by reverse transcriptase-polymease chain reaction and Western blotting analysis. Results There were characteristic changes in cartilaginous tissue in OA, such as rough cartilaginous surface, irregular arrangement, decrease in number, degeneration and necrosis of chondrocyte, and cell cluster existed. The thickness and ground-substance of cartilage in OA was thinning and uneven, and metaehromatism could be seen. The expression of IGF- Ⅱ mRNA could be seen in nucleus and cytoplasm of normal ehondrocyte in OA cartilaginous tissue. Compared to normal adult's cartilaginous tissue, the expressions of mRNA and protein of IGF-Ⅱ in OA cartilaginous tissue were decreased obviously about 75% and 63% respectively ( P 〈 0.05 ). Conclusions There is low gene expression of IGF-Ⅱ in cartilaginous tissue with OA. It maybe induce the cataplasia of cartilaginous tissue in joint.
