超声引导经直肠前列腺6点10点和12点活检术比较

摘　　要：目的:比较6点、10点、12点前列腺活检,寻找理想的前列腺癌活检方法。方法:122例怀疑前列腺癌并行经直肠超声引导下12点前列腺穿刺。结果:12点穿刺前列腺癌阳性率41.8%(51/122),其中系统6点活检阳性率(36/122)29.5%;10点穿刺前列腺癌阳性率(50/122)40.9%,12点与6点比较阳性率提高了12.3%(P<0.05);10点与6点比较阳性率提高了11.4%(P<0.05),12点与10点比较没有统计学意义(P>0.05)。结论:10点前列腺穿刺活检可提高前列腺癌的阳性率特别是早期癌的检出率,是一种理想的穿刺方法。Objective. To evaluate the diagnostic value of transrectal ultrasound - guided （TRUS） prostate biopsy 12 - core compared with 10 - core and the sextant biopsy technique and develop an optimal biopsy regimen for detecting prostate cancer. Methods： 122 patients with suspected prostate cancer were included in the study. Results： Cancer detection rates were 41.8 % （51/122）, （50/122） 40.9 %, and（36/122） 29.5 % for the 12 - core , 10 - core and sextant core biopsy protocols, respectively） The 12 - core to the sextant prostate biopsy showed a 12.3 % increase in the cancer detection rate in patients with positive prostate cancer（P 〈 0.05）. The 10- core to the sextant prostate biopsy showed a 11.4% increase in the cancer detection rate in patients with positive prostate cancer（P 〈 0.05）. 10 - core biopsy regimen compared with the sextant biopsy regimen no posi- tive significance（P 〉 0.05）. Conclusion：The 10 systematic biopsy improves the detection rate of prostate cancer without increase in the morbidity. We therefore recommend the 10 - core biopsy protocol to be the preferred method for prostate cancer especially early detection of prostate cancer.
