声学造影心肌灌注显像和多巴酚丁胺负荷超声心动图试验检测存活心肌的实验研究

摘　　要：本文应用声学造影心肌灌注显像（ＭＣＥ）和小剂量多巴酚丁胺负荷超声心动图试验（２ＤＥ－ＤＳＥ）检测６条犬的顿抑心肌，并与ＳＰＥＣＴ核素心肌灌注显像和病理对照。结果ＭＣＥ判别顿抑心肌的敏感性和特异性为８７％、１００％，２ＤＥ－ＤＳＥ判别顿抑心肌的敏感性和特异性分别为９１％和１００％。ＳＰＥＣＴ核素显像以存活分数为０．１作为标准时，判断顿抑心肌的敏感性为７４％；以存活分数为０．２为标准时，判断顿抑心肌的敏感性为４８％。To evaluate the sensitivity of myocardial contrast echocardiography(MCE)and lowdose dobutamine stress echocardiography(DSE)in detecting viable myocardium,we studied 6 canine stunning myocardial models.The results were compared with both SPECT radionuclide imaging and pathology.Results:20/23 segments of stunning myocardium were recognized by MCE.The sensitivity and specificity of detecting stunning myocardium with MCE were 87% and 100% respectively.21/23 segments of stunning myocardium were recognized by DSE.The sensitivity and specificity of detecting stunning myocardium with DSE were 91% and 100% respectively.With the standard of viable fraction 0 1,17/23 segments of stunning myocardium were detected by SPECT.The sensitivity of detecting stunning myocardium with SPECT was 74%.with the standard of viable fraction 0 2,11/23 segments of stunning myocardium were detected by SPECT.The sensitivity of detecting stunning myocardium with SPECT was 48%. Conclusions:The sensitivity of detecting viable myocardium with MCE is similar as DSE,and is much higher than SPECT.
