扩大外耳道带蒂肌骨膜皮瓣在开放式乳突根治术中的应用

摘　　要：目的探讨扩大外耳道带蒂肌骨膜皮瓣在开放式乳突根治术和开放式改良乳突根治术中的应用。方法回顾性分析62例(62耳)胆脂瘤型中耳乳突炎患者,43例行开放式乳突根治术,19例行开放式改良乳突根治术。应用扩大外耳道带蒂肌骨膜皮瓣行乳突术腔填塞术,同期行鼓室成形术9例。结果术后随访1～3年,乳突腔内被肌骨膜皮瓣完整填塞,位置良好,无脱出和坏死,无回缩现象,新形成的外耳道宽大。术后术腔完全上皮化时间为48～55d,平均40d,干耳率100%。结论扩大外耳道带蒂肌骨膜皮瓣填塞乳突术腔具有乳突术腔填塞完整,皮肤覆盖好,上皮修复快,术腔愈合时间短,术后干耳率高。手术简单、易行,便于推广。Objective To investigate the clinical value of extended pedicled myoperiosteal flap of external auditory canal in opening mastoidectomy and modified mastoidectomy. Methods From Jan. 2004 to Dec 2006, 62 patients （62 ears ） with otitis media with cholesteatoma reveived opening mastoidectomy or modified mastoidectomy. The extended pedicled myoperiosteal flap of external auditory canal was applied to all cases, and simultaneous tympanoplasty was performed in 9 cases. Results After following up for 1 to 3 years, we found that the flaps were placed rightly in the mastoidal cavity without hernia, necrosis, and retraction, and the reconstructed external auditory canals were enlaged. The time for complete reepithelialization of mastoidal cavity ranged from 48 to 55 days, with an average of 40 days. Rate of dry ear was 100%. Conclusion The application of extended pedicled myoperiosteal flap of external auditory canal to the opening mastoidectomy has advantages of complete packing of the mastoidal cavity, better reepithelialization and short healing time, and a high rate of dry ear after operation.
