摘  要:目的:评价64层螺旋CT冠状动脉血管成像(CTA)在诊断各种类型冠状动脉变异中的价值和临床应用。方法:回顾性分析617例行冠状动脉CTA检测患者的冠状动脉变异检出率,并根据CT结果对冠状动脉变异进行分类。结果:617例冠状动脉CTA受检者中,共检出冠状动脉变异78例(12.64%,78/617),其中右冠状动脉起自主动脉左冠状窦者9例,左冠状动脉起自主动脉右冠状窦者2例,右冠状动脉开口高位者2例,多开口变异者2例(并行左主干),心肌桥者59例,前降支-肺动脉瘘者2例,左右冠状动脉-肺动脉瘘者1例,右冠状动脉-左室瘘合并右冠状动脉瘤者1例。结论:冠状动脉CTA能准确评估冠状动脉变异,并可清晰显示各类病变细节,可作为无创性诊断冠状动脉变异的首选方法。Objective To evaluate the diagnostic value of 64-slice multi-detector row spiral CT coronary angiography in patients with coronary artery anomalies. Methods The value of 64-slice multi-detector row spiral CT angiography in detecting coronary artery anomalies in 617 patients was retrospectively analyzed. Results Seventy-eight cases (12.64%, 78/617) of coronary artery anomalies were detected by 64-sclice mufti-detector row spiral CT angiography. The anomalies consisted of 9 cases of right coronary artery originated from left aortic sinus, 2 case of left coronary artery originated from right aortic sinus, 2 cases of right coronary artery with high take-off origin, 2 case of multiple coronary ostia, 59 cases of myocardial bridging, 2 cases of LAD-PA fistula, 1 case of bilateral coronary artery-pulmonary artery fistula, 1 case of RCA-LV fistula with RCA aneurysm. Conclusions 64-slice multi-detector row spiral CT coronary angiography can accurately evaluate coronary artery anomalies and show details of different types of anomalies. It is recommended as the first-line non-invasive method for diagnosing coronary artery anomalies.
