摘  要:目的:利用左心瓣膜反流探讨无创测定左心腔内压力的新方法。方法:应用连续波多普勒超声心动图(CWDE)和心导管同步测定47例患者二尖瓣反流最大压差和主动脉瓣反流舒张末最大压差,结合肱动脉压的测量,计算左心房压和左心室舒张末压。结果:两种技术测定的左心室舒张末压间存在良好相关性(r=0.85,y=1.201x-0.286,P<0.001),左心房平均压与平均肺小动脉嵌顿压间高度相关(r=0.95,y=0.901x+0.154,P<0.001)。结论:CWDE能准确测定左心瓣膜反流压差。? Objective:To approach new methods of noninvasive quantification of leftside intracardiac pressure in patients with mitral and/or aortic regurgitations. Methods:In 47 patients of mitral regurgitation and partially associated with aortic regurgitation,peak mitral and enddiastolic aortic regurgitant gradients were respectively measured by simultaneously continuouswave Dopplerecho and left and right cardiac catheterization,then the left atrial and left ventricular enddiastolic pressures were calculated together with the measurement of brachial artery pressure. Results:A good correlation(r=085,y=1201x-0286,p<0001) was observed between the left ventricular enddiastolic pressures by the two techniques,there was a high correlation(r=095,y=0901x+0154,p<0001)between left atrial and mean pulmonary artery wedge pressures. Conclusion:The continuouswave Dopplerecho provides a new way for noninvasive measurement of leftside intracardiac pressures by accurately measuring the pressure gradient of leftside valvular regurgitation.
