摘  要:应用彩色多普勒超声仪探测23例伴微白蛋白尿(WM组)及26例不伴微白蛋白尿(NM组)的非胰岛素依赖型糖尿病(NIDDM)患者左心室功能及结构参数,并与尿微量白蛋白(UMA)及胰岛素敏感指数(IAI)进行相关分析。结果:与对照组比较,WM组及NM组NIDDM患者均存在左室舒张功能障碍和胰岛素抵抗,且WM组伴有左室收缩功能及结构参数异常。除外高血压患者后,WM组较NM组左室舒张功能及结构参数仍具显著性差异(P<0.05),且WM组较NM组有更显著的胰岛素抵抗和高胰岛素血症。UMA与IAI及左室功能、结构参数显著相关(均为P<0.01)。The pulsed doppler echocardiography was used to detect left Ventricular(LV) function and stracture and evaluate its relationship to urinary microalbuminuria(UMA) and insulin sensitivity index (IAI) in 23 non-insulin-dependent diabetes mellitus (NIDDM) patients with microalbuminuria (WM group) and 26 without microalbuminuria (NM group). The results showed that there were LV diastolic dysfunctions and insulin resistance in both WM group and NM group NIDDM patients, and the abnormal LV stracture param-eters and systolic dysfunction were only found in WM group. Eliminating the patients with hypertension in two groups NIDDM, the difference between WM group and NM group were sigificantly on LV diastolic func-tion and stracture parameters (P<0. 05), Hyperinsulinemia and insulin resistance were more significant in WM group than in NM group. Regression analysis showed that UMA was significantly related to the parame-ters of LV function , stracture and IAI(all P<0. 01).
