沙美特罗/替卡松粉吸入剂治疗中重度慢性阻塞性肺疾病临床观察

摘　　要：目的观察沙美特罗/替卡松粉吸入剂(舒利迭)治疗稳定期严重程度为Ⅲ～Ⅳ级慢性阻塞性肺疾病的临床疗效。方法将我院呼吸科门诊就诊的142例确诊为Ⅲ～Ⅳ级COPD患者,随机分为治疗组和对照组。对照组给予常规解痉、平喘、祛痰等治疗,治疗组在此基础上给予吸入沙美特罗/替卡松粉吸入剂(规格50/500μg)1吸/次,2次/日,疗程24周,分别对两组患者治疗前后的症状缓解、急性发作频率、肺功能指标进行观察对比,评价药物疗效。结果治疗组患者治疗前后症状及体征明显改善,两组疗效比较差异有统计学意义(P<0.01)。治疗组治疗前后FEV1、FEV1预计值%、FEV1/FVC%有显著改善,差异有统计学意义(P<0.05);而对照组各指标差异无统计学意义。治疗组患者急性发作频率明显减少,有统计学意义(P<0.05),对照组无明显变化。结论沙美特罗/替卡松粉吸入剂(规格50/500μg)治疗稳定期严重程度为Ⅲ～Ⅳ级的慢性阻塞性肺疾病患者,能有效改善症状、提高生活质量、明显改善肺功能、减少急性发作频率,值得临床上推广。Objective To observe the effects of salmeterol xinafoate and fluticasone propionate powder for inhalation （seretide） in the treatment of chronic obstructive pulmonary disease（ COPD ）. Methods 142 cases outpatient with diagnosis of grade ⅢtoⅣ COPD were randomly divided into treatment group and control group. The control group received conventional treatment,the treatment group was given inhaled seretide （50/500μg） beside conventional treatment,the dosage is one inhalation each time,twice per day,period of treatment is 24 weeks. The symptom, frequency of acute attack and index of pulmonary function of two groups were contrasted to evaluate the curative effect. Results The symptom and sign were obviously improved in treatment group, the difference between two groups had statistic significance （p 〈 0.01）. The FEV1, FEV1/PREDICTION%, FEVI/FVC% in the treatment group were improved obviously after treatment（P〈 0.05）, but the index before and after treatment hadno obvious difference in control group. The frequency of acute attack in treatment group had decreased obviously after treatment（P〈 0.05）, this index had no obvious change in control group before and after treatment. Conclusion Salmeterol xinafoate and fluticasone propionate powder for inhalation （seretide） （50/500 μg） in the treatment of moderate to severe COPD can obviously improve symptom and quality of life, ameliorate pulmonary function and reduce the frequency of acute attack.
