摘　　要：目的了解ICU目前有创机械通气患者床头抬高30-45°依从性的现状。方法采用美国危重症护理学会(AACN)推荐的床头抬高依从性调查工具表,对1185例次有创机械通气患者,从6个不同时间点:8:00、12:00、16:00、20:00、24:00、4:00对床头抬高依从性进行调查,并比较护士年资、班别及患者意识状态对床头抬高依从性的影响。结果符合床头抬高30-45°的共213例次,依从性仅17.97%,白天平均床头抬高依从性较夜晚高9.07%;护士年资、班别差异(P<0.05)有统计学意义,患者意识状态水平不同(P<0.01)有统计学意义。结论ICU有创机械通气患者床头抬高30-45°依从性低,应采取措施改善现状。Objective To study the compliance of ICU patients with invasive mechanical ventilation undergoing 30-45° bed head elevation, and find out the reasons. Methods Compliance of table recommended by American association of critical-care nurses（AACN） were used. 1185 cases with invasive mechanical ventilation were surveyed at six different points in time（within 24 hours） on the bed elevation compliance. Results 213 cases received 30-45° bed head elevation. The compliance is 17.97%. Day compliance was higher than night compliance. The relevant factors are the work of nurses age, class and pa- tients＇ consciousness（P〈0.01）. Conclusions Compliance of bed head elevation of patients undergoing invasive mechanical ventilation is low. Some measures must be adopted to change the status.
