摘 要:目的:观察臂丛下干撕脱伤和离断伤后急性期残留神经中炎性细胞的侵润和c-Fos表达情况,为臂丛损伤后疼痛的发生机制提供线索.方法:分别制作单侧臂丛下干撕脱伤和离断伤的大鼠模型,用HE染色和免疫组织化学方法观察损伤后急性期残留神经中的中性粒细胞、单核细胞侵润情况和c-Fos表达.结果:下干离断伤5h后在断口附近有少量中性粒细胞和单核细胞侵润,而相应的背根节几乎没有,c-Fos表达亦限于断口附近;下干撕脱后炎性细胞和淋巴细胞在断口处、背根节和脊神经中枢支中均大量聚集,c-Fos也不仅在断口处的Schwann细胞中有表达,在背根节和脊神经中枢支也有大量分布.结论:撕脱伤引起的炎性和Schwann细胞反应比离断伤更强烈,且范围更广,这可能是撕脱伤比其他损伤方式引起更严重神经病理痛的原因之一.AIM: To study the effect of axotomy and avulsion of the low branch of brachial plexus(LBP) on the inflammatory cells infiltration and c-Fos expression in residual nerves and to provide clues for the mechanisms of pain production following braehial plexus injury. METHODS: Unilaterally cutting and avulsion of the LBP were made in 2 different groups of rats, respectively. Infiltration of inflammatory cells was demonstrated with HE stai- ning and the expression of c-Fos were observed immunohistochem- ically. RESULTS: Five hours after the cutting of LBP, infiltra- tion of neutrophils and monocytes was concentrated in the woun- ded end of residual nerve. Cells with c-Fos expression were also located in the wound area. Following LBP avulsion, aggregation of neutrophils, monoeytes and lymphocytes were seen not only in wounded area, but also in the related dorsal root ganglia and cen- tral process of the spinal nerve. Furthermore, more numerous c-Fos positive cells in wounded area were seen in avulsion group than in cutting group. C-Fos positive cells were found in dorsal root ganglia and central process of the peripheral nerve as well. CONCLUSION: Inflammatory ceils infiltration and c-Fos expression are stronger and more extensively distributed following avulsion compared with sharp cutting. These results indicate that
