舒芬太尼复合丙泊酚靶控输注麻醉在ERCP+EST中的应用

摘　　要：目的:观察静脉注射舒芬太尼复合丙泊酚靶控输注在逆行胰胆管造影(ERCP)+十二指肠乳头切开取石术(EST)中的麻醉效果。方法:40例ASAⅠ～Ⅱ级行ERCP+EST的患者,随机分为两组,舒芬太尼复合丙泊酚靶控输注麻醉组(S+P组,n=20)和丙泊酚靶控输注麻醉组(P组,n=20)。观察两组异丙酚用量,初醒时间、准确定向时间、术中体动(程度和次数)、麻醉效果、呼吸抑制评分和HR、MAP、SpO2及术后并发症的发生情况等。结果:两组患者初醒时间、准确定向时间、呼吸抑制评分无明显差异,异丙酚用量和术中体动(程度和次数)发生率P组大于S+P组(P<0.05);血流动力学变化:两组诱导后HR、MAP较诱导前下降(P<0.05),但两组间无明显差异,进镜后恢复至手术前水平,两组患者术后并发症的发生情况无明显差异,对检查治疗过程均无记忆。结论:舒芬太尼0.1～0.15μg/kg复合丙泊酚靶控输注静脉麻醉行ERCP+EST安全、有效。Objective ：To observe the effect of surfentanyl combined with propofol given by target-controlled infusion （TCI） in endoscopic retrograde cholangio- pancreatography （ERCP）＋ endoscopic stone extraction technigue （EST）.Methods：Forty ASA grade Ⅰ-Ⅱ patients,undergoing ERCP＋EST,were randomly separated into two groups,sufentanil combined propofol target-controlled infusion anesthesia group （S＋P group,n=20） and propofol target-controlled infusion group （P group,n=20）. The recovery time,orientation time,body movement during operation,effects of anesthesia, respiratory depression score, HR, MAP, SpO2 during operation, side-effects after operation and the total amount of propofol used were recorded and compared between the 2 groups.Results：There was no significant difference in the recovery time,orientation time and respiratory depression score. The amount of propofol and body movement during operation score in P group was greater than those of S＋P group（P〈0.05）.HR,MAP in two groups were decreased after sedation begun,compared with presedation （P〈0.05）,there was no significant difference between the two groups. But it was resumed pre-surgery after operation. Two groups of patients with post-operative complications occurred in no circumstances differences,the patients in the treatment had no memory to operation. Conclusion：It is safe and available for surfentanil（0.1-0.15 μg/kg ）combined with propofol TCI anesthesia in ERCP＋EST.
