摘 要:目的探讨肝移植患者血小板及其参数检测的临床意义以及与预后的相关性。方法采用全自动血液分析仪(拜耳ADVIA2120型)分别测定127例肝移植术前患者、81例肝移植术后(90d)无出血患者、12例肝移植术后有出血患者以及150例健康对照者的血小板及其参数等相关指标。结果1、与健康对照组比较:(1)肝移植术前组血小板计数(PLT)、平均血小板体积(MPV)、血小板压积(PCT)、血小板分布宽度(PDW)差异有统计学意义(P<0.01;P<0.05);(2)肝移植术后无出血组PLT、MPV、PCT、PDW差异无统计学意义(P>0.05);(3)肝移植术后有出血组PLT、MPV、PCT差异有统计学意义(P<0.01;P<0.05),PDW差异无统计学意义(P>0.05)。2、与肝移植术前组比较:(1)肝移植术后无出血组PLT、MPV、PCT差异有统计学意义(P<0.01),PDW差异无统计学意义(P>0.05);(2)肝移植术后有出血组PLT、MPV、PCT、PDW差异无统计学意义(P>0.05);3、肝移植术后有出血组与无出血组比较:PLT、MPV、PCT差异有统计学意义(P<0.01;P<0.05),PDW差异无统计学意义(P>0.05)。结论血小板及其参数相关指标的检测:(1)可配合临床观察肝移植术后疗效以及判断病情的变化。(2)间接反映肝移植术后PLT的恢复功能。Objective To explore the clinical significance of platelet and correlative parameters in liver transplant patients. Methods ADVIA 2120 was used for detection of liver transplantation patients' platelet and correlative parameters, including 127 preoperative patients, 81 postoperative patients (90 days) without hemorrhage, 12 postoperative patients with hemorrhage and control group. Results 1. Compared with control group's PLT,MPV, PCT and PDW: (1)There was a statistical significauce in preoperative patients ( P 〈0.01, P 〈0.05). (2) but no significant differences in postoperative patients without hemorrhage( P 〉 0. 05). (3) There was also a statistical significance in postoperative patients with hemorrhage ( P 〈0. 01, P 〈0. 05) except PDW( P 〉0. 05). 2. Compared with the preoperative patients' PLT,MPV,PCT and PDW: (1) There was a statistical significance in postoperative patients without hemorrhage( P 〈 0. 01 ) exceptj PDW( P 〉 0. 05 ). (2) but no significant differences in postoperative patients with hemorrhage( P 〉0. 05). 3. Compared with the postoperative patients without hemorrhage, PLT,MPV, PCT in hemorrhage patients has significant differences ( P 〈0. 01, P 〈0. 05) except PDW( P 〉0. 05). Conclusion The detection of platelet and correlative parameters could help clinical doctors to observe the efficacy of liver transplantation, judge the changes of disease; it also reflect the function of PLT after the operation.
