摘  要:目的探讨谷氨酰胺对脓毒症小鼠炎性反应及生存率的影响。方法昆明小鼠45只,随机分为假手术组(CON)、手术组(CLP)、谷氨酰胺组(GLN),每组15只。采用盲肠结扎穿孔法(CLP)制备脓毒症模型,术后即刻尾静脉注射谷氨酰胺0.75g/kg(GLN组)或等体积0.9%氯化钠溶液(CON和CLP组)。6h后每组各取5只,收集血清,腹腔灌洗分离巨噬细胞,采用酶联免疫吸附试验(ELISA)测定血清和巨噬细胞中肿瘤坏死因子(TNF)-α、白细胞介素(IL)-6、IL-10水平;另外每组各取10只观察术后5d生存率。结果CLP和GLN组血清中TNF-α、IL-6、IL-10水平均显著高于CON组(P值分别<0.05、0.01);GLN组血清TNF-α水平显著低于CLP组(P<0.05),两组间血清IL-6、IL-10的差异均无统计学意义(P值均>0.05)。GLN组巨噬细胞中TNF-α、IL-6水平均显著低于CLP组(P值分别<0.01、0.05),3组间IL-10水平的差异无统计学意义(P>0.05)。GLN组小鼠的存活时间显著长于CLP组(P<0.05)。结论谷氨酰胺可以抑制脓毒症小鼠的炎性反应,降低其病死率。Objective To investigate the protective effects of glutamine (Gin) on septic mice. Methods Forty-five Kunming mice were randomly divided into three groups (n = 15 each): sham-operated group (CON), sepsis model group (CLP), and Gin-treated group (GLN). Sepsis was induced by cecal ligation and puncture. Either glutamine (0.75 g/kg) or saline was administered immediately after surgery via single tail-vein injection and the survival rate was observed. Macrophages were harvested by peritoneal lavage at 6 h. Both intracellular and serum cytokines were assayed via ELISA. Results Serum IL-6 and IL-10 levels were similar between two groups (all P〉0.05) ; the serum level of TNF-α in GLN group was significantly lower than that in CLP group (P〈0.05). Intracellular TNF-α and IL-6 levels were also significantly lower in GLN group compared with CLP group ( P〈0.01, 0.05), but the intracellular IL-10 level was similar among the 3 groups (P〉0.05). Survival rate at 5 days post treatment was significantly higher in GLN group (30%) than that in the GLP group (10%, P〈0.05). Conclusion Gin treatment exerts an anti-inflammatory effect in vivo during sepsis. The anti-inflammatory effect of Gin is also associated with improvement of this septic mouse survival.
