摘 要:目的:评价深度镇静(咪达唑仑联合氯胺酮镇静)下行儿童埋藏牙拔除术的效果,探讨其利弊。方法:选择2005-03~2007-03需拔除埋藏牙的患儿187例(年龄6~12岁)。术前由专业麻醉医生给予咪达唑仑0.1 mg/kg+氯胺酮1 mg/kg肌注。观察镇静起效时间、维持时间并给予患儿镇静评分。结果:187例患儿在深度镇静下均能很好的配合完成手术,且术中及术后未见任何并发症发生。起效时间为3~5 min,平均3.5 min;维持时间为45~50 min,平均46.5 min。镇静评分均在3分以上,其中4分者131例,5分者56例。结论:在深度镇静下行儿童埋藏牙拔除术,麻醉药物起效快,代谢时间短,副作用少,效果显著,值得临床推广。Objective:To assess the effects of deep sedation (sedation with ketamine/midazolam) in extraction of children's imbedded teeth and evaluate its advantages and disadvantages. Methods: 187 children aged from 6 to 12 with embedded teeth were chosen for the study. Intramuscular midazolam 0.1 mg/kg with ketamine 1 mg/kg were administered by an anesthetist before teeth extraction. Maximum level of sedation time, duration time and sedation levels were recorded. Results: All the children could endure the operation under the deep sedation. No complication happened during or after the operation. It took 3 -5 min (averaged 3.5 min) to achieve the maxi- mum level of sedation, which could keep 45 -50 min ( averaged 46.5 min). Sedation levels were all above 3 in all cases, among which 131 got 4 points and 56 got 5 points. Conclusion: Deep sedation is effective, quick and with rare side effects. Deep sedation is recommended in children's imbedded teeth extraction.
