产前产时高危儿系统管理的效果评估

摘　　要：目的探讨围生期高危儿系统管理新模式中产前、产时的措施及评估其效果。方法建立围生期高危儿系统管理模式并开展实施。回顾性调查自开展管理新模式以来(1999—2007年)全部住院分娩资料及相关新生儿资料,分析与高危儿生活质量密切相关的主要指标如围生期死亡率、窒息率、缺血缺氧性脑病(H IE)、颅内出血(ICH)、胎粪吸入综合征(MAS)以及极低出生儿的存活率等。结果1999年—2007年分娩数及高危妊娠数虽然逐年增加,但围生儿死亡率呈明显下降趋势,窒息率下降更明显,尤其是近3年的重度窒息率接近零,发生HIE、ICH、MAS的机率总体呈下降趋势,特别是HIE发生率由1999年的7.1‰降至2007年的1.8‰,MAS也由4.6‰降至3.1‰,ICH6.8‰降至5.1‰。极低出生体重儿生存率由1999年的71.4%升至近3年的90%以上。结论产前、产时围生期高危儿系统管理新模式的开展,能有效地提高围生儿的生活质量。Objective To study the new management model of perinatal high-risk infants system during antipartum and intrapartum and assess its effectiveness. Methods New Perinatal high-risk infants system management model was established and acarried out. All related datum during delivery and neonate were surveyed retrospectively since new model was carried out （ 1999 - 2007 ） . The author analyse those main index related to the quality of life such as perinatal mortality, the rate of asphyxia, Hypoxic ischemic encephalopathy （HIE）, Intracranial hemorrhage （ICH）, Meconium aspiration syndrome （MAS） and the survival rate of very low birth weight infants （VLBW） . Results Although the delivery of high-risk pregnancy increased from 1999 to 2007, the perinatal mortality decreased obviously, the rate of asphyxia decresed more obviously. In 1st 3 year, the rate of severe asphyixa close zero. The rate of HIE, MAS and ICH decreased from 7. 1‰, 4.6‰, 6. 8‰ to 1.8‰, 3.1‰, 5. 1‰ respectively. The survival rate of VLBWI increased from 71.4% to above 90%. Conclusion The new management model of perinatal high-risk infants system can improve effectively the quality of life on perinatal.
