脊髓康治疗犬脊髓损伤后痉挛性膀胱的实验研究

摘　　要：目的:观察脊髓康对犬脊髓损伤后痉挛性膀胱的治疗作用,并与选择性骶神经根切断术进行对照。方法:雄性家犬30只随机分为6组(n=5),A组:正常组;B组:模型组;C组:切断S1神经根;D组:切断S1、S2神经根;E组:切断S1～S3神经根;F组:脊髓康组,造模后予脊髓康。造模成功1周后行选择性骶神经根切断术。观察犬一般表现,存活率,排尿量及阴茎勃起情况;实验结束取损伤脊髓作病理学分析。结果:A组活动能力正常,B、C、D、E组双后肢无任何活动,F组活动能力较强。手术后第1、第7天A组勃起功能正常,B、F组勃起率相似;C、D、E组勃起率随骶神经根切断数量的增加而依次降低。C、D、E及F组术后排尿量高于B组而低于A组。病理结果:脊髓康组犬脊髓病理变化程度较轻。结论:脊髓康可改善脊髓损伤犬生存质量;可缓解逼尿肌、括约肌痉挛,改善脊髓损伤后痉挛性膀胱的贮尿功能及降低尿道出口阻力;可促进损伤脊髓的恢复,减轻脊髓水肿。阴茎勃起功能与S3神经关系密切,但存在个体差异。Objective：To observe the effect of Jisuikang in treating spastic bladder （SB） after spinal cord injury （SCI） compared with selective neurotomy of sacral nerve in dogs. Methods：Thirty male dogs were randomly divided into six groups （n =5） ： group A （normal control）, group B （model group）, group C （S1 nerve root cutting off）, group D （S1 and S2 nerve roots cutting off）, group E （S1 , S2 and S3 nerve roots cutting off） and group F （Jisuikang group）. Sacral nerve neurotomy was selectively performed one week after models were established. General condition, survival rate, urinary output and penile erection were observed. Spinal cord histopathologic examination was done after the treatment. Results：The motion activity was normal in group A, better in group F, and there was no activity in group B, group C, group D and group E in both posterior limbs. In group A and group F, the survival rate was higher than the rest groups. The erection rate was normal in group A and similar in group B and group F. In group C, group D and group E, the erection rate decreased with the number of cut off sacral nerve roots increased. The urinary output after operation was higher in group C, group D, group E and group F compared with group B but lower than group A. Mild pathological changes of spinal cord tissue were observed in Jisuikang group. Conelusion：Jisuikang can evelate the dogs＇ life quality after spinal cord injury, relieve spasm of the detrusor muscle and sphincter, improve storing urine function of spastic bladder and degrade the resistance of urethra exit; also, it can lessen edema of spinal cord and promote recovery of injured spinal cord tissue. The dogs＇ penile erection function has close relationship with S3 nerve, but has individual differences.
