冠心病病人室性心律失常发生前后HRV变化  

Changes of HRV Before and After the Onset of Ventricular Ectopic Activity in Patients with Coronary Heart Disease

摘　　要：将６３例冠心病病人分为低危组（０～Ⅱ级）和高危组（Ⅲ级以上），观察了全天每小时ＨＲＶ变化，并将有室性心律失常者分为室早组（Ⅰ、Ⅱ）、多型室早组（Ⅲ级）、成对室早组（Ⅳａ）、室速组（Ⅳ），对室性心律失常高峰前后的心律变异性（ＨＲＶ）进行了比较，结果显示：冠心病人高危组ＨＲＶ较低危组低，室速组、成对室早组室性心律失常高峰前ＨＲＶ较室性心律失常高峰后明显降低，差异有显著意义（Ｐ＜００１和Ｐ＜００５）。提示低危组与高危组间自主神经活动存在差异，室速。Hourly heart rate variability(HRV) in the whole day was assessed in 30 patients with ventricular ectopies of Lown's grade 0 Ⅱ (lower risk group, Group C1) and 32 patients with ventricular ectopies of Lown's grade Ⅲ Ⅳ (higher risk group, Group C2 ). HRV(LF,HF) in one hour before the peaky onset of ventricular ectopic activity (LFb,HFb) and that after the peaky onset of ventricular ectopic activity (LFa,HFa) were obtained from 46 patients of them who were with ventricular ectopic activity and were classified into four groups: Group A1(Lown's grade Ⅰand Ⅱ), Group A2(Lown's grade Ⅲ ), Group A3(Lown's grade Ⅳa), Group A4(Lown's grade Ⅳb). Hourly LF in Group C2 were statistically significantly decreased in comparison with Group C1 (P<0 01) ,but the markedly decreased HF in every hour in Group C2 (P<0 01and P<0 05) was from 23PM.to 8AM. HFb were statistically significantly lower than HFa in Group A3 and A4(P<0 05),LFb was significantly lower than LFa only in Group A4(P<0 01). The results suggested that autonomic nervous activities between higher risk group and lower risk group were markedly different, the onset of ventricular tachycardia and couplet episodes were associated with the decreased HRV.
