摘 要:本文旨在研究鉴别缺血性心肌病(ICM)与扩张型心肌病(DCM)的超声鉴别诊断方法。应用二维超声心动图对70例ICM及70例DCM的心脏形态学结构进行对比研究,优选7项指标建立鉴别ICM与DCM的Fisher判别式。结果两组RVOT、AO、LA、RV、IVSd、LVPWd和LVDd差别极为显著,以Fisher判别式46(RVOT+RV)+261LA/AO+288LVDd/(IVSd+LVPWd)-3758<0为ICM,≥0与DCM,判别总符合率达978%,F检验P<001。提示Fisher判别式Z=46(RVOT+RV)+261LA/AO+288LVDd/(IVSd+LVPWd)-3758是鉴别ICM与DCM的简便、准确、无创的方法。The purpose of this study was to find the differential method for ICM form DCM with two dimensional echocardiography.Two dimensional echocardiography was used to analysis the cardiac morphology in 70 cases of ICM and 70 cases of DCM. Seven indinces were chosen and Fisher discriminative analysis was used to obtain the discriminant.The results showed that ICM and DCM were significantly different in these indices RVOT,AO, LA, RV, LVDd,IVSd and LVPWd.Take Fisher discriminant 4 6 (RVOT+RV)+26 1 LA/AO+28 8 LVDd(IVSd+LVPWd)-375 8 <0 as ICM,≥0 as DCM.The overall diagnostic coincidence rate was 97 8%.F test P<0 01.This result suggests that the Fisher discriminant Z=4 6 (RVOT+RV)+26 1 LA/AO+28 82 VDd/(IVSd+LVPWd) 375 8 was a simple, accuracy, noninvasine differential method for ICM from DCM.
