闭合复位经皮穿针内固定治疗老年性肱骨近端骨折

摘　　要：目的回顾性研究闭合复位并经皮穿针固定、康复训练治疗老年性肱骨近端骨折的技术方法,探讨其安全性、临床疗效。方法将78例患者随机分为研究组和对照组,研究组47例实施经皮穿针内固定治疗及术后康复训练,对照组31例行非手术治疗。结果随访8～20周,研究组全部骨性愈合(100%),愈合率明显高于对照组(80.05%,P<0.05);愈合时间(11.2±1.4)周短于对照组(15.8±1.1)周(P<0.05);肩关节评分优良率(91.74%)明显高于对照组(55.68%,P<0.01)。结论闭合复位经皮穿针内固定具有创伤小、固定可靠、骨折愈合率高等优点,术后肩关节功能恢复良好,是一种治疗老年性肱骨近端骨折效果明显、并发症少的微创技术方法。Objective To study the technique of closed reduction and percutaneous pinning internal fixation combined rehabilitation training in the treatment of proximal humerus fracture for senile cases, and to investigate the safety and therapeutic effect. Methods Seventy-eight senile patients were divided into the study group and the control group randomly. The study group included 47 cases treated by closed reduction and pereutaneous pinning internal fixation,and the control group included 31 cases treated by non operation. Results In the 8-20 weeks of follow up, all fractures in the study group got synostosis, the healing rate was 100% , but 80.05% for the control group（P〈0.05）. The healing time of the study group[（11. 2±3.4） weeks] was shorter than that of the control group [（15.8±4.1）weeks] （P〈0.05）. Furthermore,for Neer score of the shoulder joint function,the fineness rate in the study group（91.74%） was higher than that of the control group（55.68%）（P〈0.01）. Conclusion For senile case,the operation on proximal humerus fracture by closed reduction and percutaneous pinning internal fixation can make diminutive trauma and get reliable fixation and high union rate,and the shoulder joint function can get reablement well. So it is a satisfactory micro-traumatic technique.
