摘  要:目的研究选择性COX-2抑制剂对LDH早期炎性反应的作用,以及对突出椎间盘组织吸收的影响。方法家兔32只,随机分为药物组和空白组,每组16只,Yoshida法制作L5-L6硬膜外椎间盘突出模型,药物组以5mg/kg/dayCelecoxib的剂量与饲料混合后喂养,空白组则喂养普通饲料,4周后全部处死,每组各取8个L5-L6节段椎间盘用于突出椎间盘面积和PGE2含量的测定;剩余每组各8个L5-L6节段,用于免疫组化检测CD-68巨噬细胞。结果药物组PGE2的含量36.01±6.90pg/mg,空白组为61.07±8.75pg/mg(P<0.05);药物组椎间盘突出面积为2.20±0.18mm2,空白组为1.94±0.11mm(2P<0.05);药物组有2个(25%)椎间盘有较多的巨噬细胞(++),而空白组有4个(50%)椎间盘是(++)。结论选择性COX-2抑制剂减轻突出组织诱发的炎性反应,可能阻碍LDH重吸收过程,COX-2抑制剂对LDH早期的影响值得进一步研究。Objective To analyse the inflammatory reaction in the early stage of lumbar disc tissue under selective cyclooxygenase-2 (COX-2)inhibitor therapy,and to assess the influence of COX-2 inhiitor on regression of herniated discs. Methods Thirty two rabbits were used and divided randomly into drug group and non-drug group,sixteen rabbits in each group. A new disc herniation model was made by Yoshida's method at L5-6 discs,the drug group were given COX-2 ihhibitor (Celecoxib)5mg/kg/day together with feeds,all rabbits were sacrificed after 4 week,then 8 of the L5-6 discs were harvested in each group,the size of the herniated discs was measured using a special way and PGE2 was detected by radioimmunoassay. For the left eight L5-6 discs in each group,5 um thin sections were stained with monoclonal antibodies CD-68 for macrophages. Resuits The content of PGE2 in drug group was 36.01±6.90pg/mg,61.07±8.75pg/mg (P〈0.05);The size of herniated disc in drug group was 2.20±0.18mm^2,and 194±0.11mm^2 in non-drug group (P〈 0.05). Macrophages were detected in all of the eight non-drug herniated lumbar disc tissue,in 4 of 8 discs(50%)were found abundant cels(++),2 discs (25%)were (++)in the drgu group,and no- macrophages were observed in one discs. Conclusion Celecoxib obviously relieves inflammation induced by disc herniation,but may also suppress the spontaneous regression of herniated discs which maybe due to less macrophages infiltrating. However,the influence of COX-2 inhibitors on lumbar disc herination during the early stage needs furher study.
