自体骨髓干细胞移植与通脉化瘀汤合用治疗糖尿病足溃疡的研究

摘　　要：目的探讨自体骨髓干细胞移植与通脉化瘀汤合用治疗糖尿病足的疗效。方法选择45例因慢性下肢缺血引起糖尿病足患者,分为常规治疗组(A组)15例,共30条患肢,自体骨髓干细胞移植治疗组(B组)15例,共28条患肢,自体骨髓干细胞移植与通脉化瘀汤合用治疗组(C组)15例,共29条患肢。治疗12周,观察各项指标,对疗效进行评估。结果A组治疗前后各项指标比较差异无统计学意义(P>0.05)。B组及C组治疗后较治疗前肢体疼痛、冷感、间歇跛行、皮肤温度、踝臂指数均有改善,差异有统计学意义(P<0.05)。C组治疗后在缓解肢体疼痛、冷感、间歇跛行、皮肤温度方面与B组治疗后比较更有效,差异有统计学意义(P<0.05)。结论自体骨髓干细胞移植技术治疗因慢性下肢缺血引起糖尿病足是有效的方法,与中药通脉化瘀汤合用可以提高自体骨髓干细胞移植技术的疗效。Objective To probe the curative effect of the combination of autologous bone marrow stem cell transplantation and Tongmai Huayu soup on diabetic foot. Methods 45 patients with diabetic foot caused by chronic lower limb ischemia were randomly divided into regularly treated group （group A） including 15 cases with 30 affected limbs, and autologous bone marrow stem cell transplantation treated group （group B）including 15 cases with 28 affected limbs, and combination of autologous bone marrow stem cell transplantation and Tongmai Huayu soup treated group （group C）including 15 cases with 29 affected limbs.The parameters and curative effect was evaluated after 12 weeks＇ treatment. Results There was no statistical significance of parameter in group A（P〉0.05）. It showed improvement in limb ache, cold feeling, intermittent limping, the ankle-brachial index, skin temperature in group B and C （P〈0.05）.The curative effect of group C on alleviating limb ache, cold feeling, intermittent limping, skin temperature was more effective than that of group B. There was a statistical significance （P〈0.05）. Conclusion Autologous bone marrow stem cell transplantation is an effective method for treating diabetic foot caused by chronic lower limb ischemia. The combination with Tongmai Huayu soup improves the curative effect of autologous bone marrow stem cell transplantation.
