改良Parks手术在低位直肠癌保肛术中的应用

摘　　要：为探讨改良Parks术式在低位直肠癌保肛术中的应用价值，回顾性分析48例低位直肠癌患者行根治性全直肠切除后，重建直肠瓣、直肠角，用常规缝合法作乙状结肠与肛管吻合。全组无手术死亡病例，术后出现吻合口狭窄1例，肛周粪渍性湿疹18例（18／48），无其他并发症。根据Wexner失禁评分调查问卷，术后3个月、6个月和12个月评分分别为7．8，4．5和1．1（0分正常，20分完全失禁）。局部复发率6．25％（3／48）。结果表明，只要严格掌握手术适应症，改良Parks低位直肠癌保肛术是一种安全有效的方法。Forty eight patients with low rectal cancer underwent the procedure of total radical resection of the whole rectum,reconstruction of rectal valve ＆ angle,and sigmoid ＆ anal canal anastomosis by routine suture. Through retrospective analysis of these clinical data, it was found out that there was no case of death due to this procedure, that there was one case with postoperative anastomotic stricture, and that there were 18 cases（18/48） with perianal eczema due to stool soak. No other complications were found. According to Wexner incontinence score scales, the score of the patients at postoperative 3,6 and 12 month was 7.8,4.5 and 1. 1 respectively（0= normal, 20 = complete incontinence of stool）. The local recurrence rate was 6.25% （3/48）. It is concluded that this improved Parks operation in radical resection ＆ sphincter preservation of low rectal cancer is safe and effective so long as the indication is strictly taken into consideration.
