心绞痛患者血清缺血修饰清蛋白水平的变化及其临床意义

摘　　要：目的观察心绞痛患者血清缺血修饰清蛋白（isehemia modified albumin，IMA）浓度的改变并探讨其临床意义。方法观察对象88例分为三组：不稳定性心绞痛组34例，稳定性心绞痛组24例，正常对照组30例。用全自动生化分析仪法测定血清IMA并行超声心动图检查评价室壁运动和心功能。结果①不稳定性心绞痛患者的血清IMA水平（77．8±11．7 ABSU／ml）明显高于稳定性心绞痛（64．1±7．6 ABSU／m1）和对照组（63．6士6．8ABSU／m1）（P〈0．01）；而稳定性心绞痛的患者与对照组之间无统计学差异（P〉0．05）。②在不稳定性心绞痛的患者中，左室射血分数（LVEF）与血清IMA水平呈明显负相关（r=-0．686，P〈0．001）；LVEF减低（LVEF〈50％）的不稳定性心绞痛患者血清IMA水平（92．1±6．3 ABSU／ml）明显高于LVEF正常（LVEF≥50％）（72．6±8．5 ABSU／ml）的患者（P〈0．01）。结论血清IMA浓度在不稳定性心绞痛的患者中明显升高，且升高水平与LVEF异常有一定的相关性。Objective This study addressed to compare ischemia modified albumin concentrations in patients with unstable angina and stable angina,and to investigate the clinical significance. Methods Eighty-eight subjects were divided into three groups：34 patients with unstable angina, 24 patients with stable angina,and 30 healthy subjects as control. Ischemia modified albumin concentrations were measured with auto-biochemical. Analysis meter technique assay and echocardiograms were detected in all patients to measure left ventricular ejection fraction （LVEF） and to evaluate ventricular wal-motion. Results ①Ischemia modified albumin concentrations were increased in patients with unstable angina （77.84±11.7 ABSU/ ml） compared with those of stable angina （64.1±7.6 ABSU/ml） and control subjects（63.6±6.8 ABSU/ml）,P〈0. 01, respectively. However,there was no significant difference between the patients with stable angina and control subjects ,P〉 0. 05. ②Ischemia modified albumin concentrations were correlated with LVEF in patients with unstable angina（r=- 0. 686,P〈0. 01）,and increased significantly in patients with abnormal LVEF （LVEF〈50 %, 92.1 ± 6.3 ABSU/ml） compared with patients with normal （LVEF≥50% ,72.6±8.5 ABSU/ml）（P〈0. 01）. Conclusion Ischemia modified albumin concentrations are increased significantly in patients with unstable angina;moreover,the levels of ischemia modified albumin are correlated with abnormal LVEF,which may have a clinical significance to evaluate the risk of patients with unstable angina.
