68例老年股骨粗隆间再骨折的原因分析和预防对策

摘　　要：目的通过分析老年股骨粗隆间再骨折的致伤因素,为预防多次骨折提供依据。方法查阅2000-2007年某医院512例粗隆间骨折住院病历,对其中再次骨折的68例患者两次骨折的年龄分布、骨折类型、致伤因素及全身情况进行分析。结果68例患者初次及再次骨折的平均年龄分别为73.6岁和76.7岁。跌倒所致初次粗隆间骨折36例(52.9%),而致再次骨折49例(72.1%)。再次骨折患有高血压及心脏疾病者31例(45.6%),患有糖尿病者28例(41.2%)。16例接受骨密度测定,骨质疏松明显。结论跌倒是股骨转子间再骨折最主要的外部致伤因素,精神障碍、老年痴呆、认知能力的下降等是跌倒的危险因素,应当积极治疗骨质疏松,加强老年患者的护理,正确指导患者进行自我锻炼,预防老年患者粗隆间再骨折。Objective To provide evidence for how to prevent intertrochanteric fractures constantly through analyzing the causes of intertrochanteric fracture. Methods Totally 512 cases of intertrochanteric fractures in a hospital from 2000 to 2007 were referred to. Among them,68 patients experienced re-fractures,and their ages,types of fractures,causes of intertrochanteric fracture and body conditions at the twice fractures were studied. Results Among the 68 cases,the average ages at their first fracture and re-fracture were 73.6 and 76.7. Respectively,36 patients（52.9%） first fractured from fall,49patients （72.1%） second fractured after fall. And 29 patients（42.6%） complicated with heart disease and hypertension at first fracture and 31 patients（45.6%） at second fracture, 22 patients（32.4%） complicated with diabetes at first fracture and 28 patients（41.2%） at second fracture. Patients were recognized obvious osteoporosis by bone density measuring. Conclusions Fall was the main external causes of intertrochanteric fracture. Mental disorders,Alzheimer＇s and cognitive decline were risk factors of fall. Osteoporosis was the internal causes of intertrochanteric fracture. Heart disease,hypertension and diabetes were high-risk factors of re-fractures. Therefore,treating osteoporosis,strengthening the care of elderly patients and guiding patients＇ self- exercising correctly were the key points to prevent the elderly＇s intertrochanteric re-fractures.
