河南省城市社区慢病综合防治试点高血压干预效果评价

摘　　要：[目的]了解河南省城市社区慢病综合防治试点经过5年的社区综合干预之后,该社区人群血压水平以及高血压相关知信行状况的改善情况,评价社区综合干预效果。[方法]在干预前、后采用分层多阶段抽样方法随机抽取该社区15～69岁常住居民进行3次入户调查,对该人群的血压水平、高血压患病率、行为危险因素以及高血压相关知信行状况进行调查研究。[结果]经协方差分析,在消除年龄因素的影响后,不同年度的收缩压水平差异有统计学意义(F=9.44,P﹤0.05);经Cochran-Armitage趋势检验,吸烟率、饮酒率、被动吸烟率有了明显下降(均有P﹤0.05);体育锻炼率有了明显提高(P﹤0.05);高血压相关知信行状况有了明显改善;建立Logistic回归模型,性别、年龄、不良婚姻、被动吸烟以及职业是影响这一目标人群血压异常的重要危险因素。[结论]该社区人群的健康状况以及高血压相关知信行状况有所改善,针对该群体的高血压综合干预措施是切实可行且已初显成效的。[ Objective]To evaluate the effect of community-based integrated intervention for hypertension in pilot sites of Henan province, and evaluate the improvement of the hypertension related knowledge, attitude, and practice in residents in Henan so as to evaluate the effectiveness of integrated intervention, [ Methods] Before and after intervention, the stratified multiple-stage sampling method was used to select residents aged 15-69 years and in-home investigation was performed. The blood pressure, prevalence of hypertension, behavioral risk factors and hypertension related knowledge, attitude, and practice were investigated. [ Results ] Overall, the level of systolic pressure in target population showed to be declined after adjusting by age （ F=9.44, P 〈 0.05）. The descending tendency was seen in the rates of smoking, drinking and passive smoking by Cochran-Armitage trend test （P〈 0,05）. After intervention, the rate of physical training showed to be significantly increased （P 〈 0.05）. Hypertension related KAPs also presented to be significantly improved （all P 〈 0.05）. The results of logistic regression analysis showed that the major risk factors which had impact on the hypertension were gender, age, physical activity, negative marriage status and passive smoking exposure. [Conclusion] The health status and hypertension related KAPs showed to be greatly improved, and community-based integrated intervention in terms of hypertension was feasible and effective.
