经尿道气化电切术与电切术、电烙术治疗膀胱白斑的比较研究

摘　　要：目的::探讨治疗膀胱白斑的有效方法,提高治疗水平。方法:对105例经病理活检证实的膀胱白斑患者,采用单盲随机方法,分别运用经尿道膀胱白斑气化电切术、膀胱白斑电切术及膀胱白斑电烙术进行治疗,并对治愈率、有效率、无效率、复发率等指标进行比较分析。结果:经尿道气化电切术组中治愈22例,好转1 2例,无效1例,术后复发2例;经尿道电切术组中治愈17例,好转14例,无效4例,术后复发8例;经尿道电烙术组中治愈13例,好转12例,无效10例,术后复发11例。组间比较,三种手术方式的疗效差异有统计学意义(P=0.027)。术后疾病的复发率差异有统计学意义(P=0.024)。结论:经尿道膀胱白斑气化电切术治疗膀胱白斑具有较高的治愈率和较低的复发率,是治疗膀胱白斑较好的手术选择。Objective：To study the treatment of bladder leukoplakia and to improve efficacy of the intervention. Methods： 105 patients with bladder leukoplakia confirmed by pathological biopsy were treated by transurethral electrovaporization, electro-resection and electrocautery using a single blind random approach. A comparative analysis on the cure rate, the efficient and inefficient rate, and the recurrence rate was perfomed among groups. Results：Of the 35 patients in transurethral electrovaporization group, 22 patients were cured, 12 patients were improved, 1 patient was incured, and 2 patients were recurrent. In transurethral resection group, 17 patients were cured, 14 patients were improved, 4 patients was incured, 8 patients were recurrent. In electrocautery group, 13 patients were cured, 12 patients were improved, 10 patients were incured, and 11 patients were recurrent. There was a significant difference among the three groups in cure rate（P=0. 027）. Postoperative recurrence rate of those three groups were 5.71 %, 22.86 % and 31.43 % respectively. There was a significant difference among the three groups in postoperative recurrence rate （P= 0. 024）. Conclusions：Transurethral electrovaporization for bladder leukoplakia is a more effective intervention with higher cure rate and lower recurrence rate compared eletro-resection and electrocautery.
