低剂量吗替麦考酚酯在直系亲属活体肾移植中的应用

摘　　要：近年来,国内亲属活体肾移植呈明显上升趋势,鉴于供受体之间良好的组织相容性、排斥反应也相对较少,有学者提出适当减少免疫抑制剂剂量,但目前对此尚无一致及具体的方案。为了探讨低剂量的免疫抑制剂方案应用的可行性,使活体肾移植技术更加安全、经济,对厦门大学附属东南医院具有直系血缘关系、亲属活体肾移植患者56例,活体肾移植后2周起实施低剂量吗替麦考酚酯分散片的免疫抑制方案,人/肾存活率分别均为96.4%(54/56)和98.2%(55/56)。排斥反应5例,其中2例为移植后2周内发生。肺部感染2例、带状疱疹1例、水痘1例,贫血1例、腹泻3例。结果证明应用该治疗方案疗效满意,排斥反应发生率并无增加、而肺部感染等并发症明显减少,而且节省了费用。Recent years, the number of patients performing living renal transplantation of relatives shows increasing trend in China. Because of the good histocompatibility between donor and recipient, as well as tess rejection, some scholars suggest appropriate decrease the dosage of immunosuppressive agents. But so far there is no consistent and concrete scheme. To investigate the feasibility of application of low dosage immunosuppressive agents, and to make living kidney transplantation safe and economic, fifty-six recipients who were from Southeast Hospital Affiliated Xiamen University and received living kidney transplantation of lineal consanguinity were selected in this experiment. They were received low dosage mycophenolate mofetil therapy from two weeks after transplantation. The survival rates of recipients and kidneys were 96.4% （54/45） and 98.2（55/56）, respectively. There were five recipients of rejection, and two of them occurred in the first two weeks after transplantation. There were two cases of pulmonary infections, one case of herpes zoster, one case of varicella, one case of anemia, three cases of diarrhea. The result confirms the efficacy of the treatment scheme were satisfied, for the rate of rejection events being not increasing while the complications such as pulmonary infection being decreasing and less expenditure.
