摘  要:目的探讨掌侧锁定加压钢板(LCP)结合Kapandji技术治疗不稳定型Colles骨折的方法及其效果。方法回顾性分析掌侧LCP结合Kapandji技术治疗的12例Colles骨折患者,比较手术前后掌倾角、尺偏角、关节面塌陷、桡骨短缩及关节功能等,初步评价其,临床疗效。结果经随访9—24个月(平均17个月),本组患者术后掌倾角、尺偏角、关节面塌陷、桡骨短缩均获明显改善,桡骨远端影像学评估采用Stewart改良的Sarmiento评分,其中优9例,良3例;腕关节功能采用Gartland—Werley功能评分,其中优2例,良7例,可3例。结论对不稳定型Colles骨折,掌侧LCP结合Kapandji技术是一种安全有效的治疗方法,可有效防止复位丢失,减少结构性植骨,允许早期关节活动,避免对肌腱激惹等并发症。Objective To explore the method and evaluate the effect of sion plate(LCP) fixation combined with Kapandji technique for unstable Colles' volar locking compresfractures. Methods Twelve patients with unstable Colles' fracture were treated with volar LCP combined with Kapandji techniques, whose effects were evaluated through comparing preoperative and postoperative volar tilt, radial inclination,articular setoff, radial shortening and wrist function. Results All cases were followed up from 9 to 24 months ( average 17 months). The volar tilt, radial inclination, articular set - off, radial shortening and wrist function of all patients were improved remarkably. Nine patients achieved excellent and 3 good according to Sarmiento scale( modified by Stewart) in the radiological manifestation. Clinically ,2 patients displayed excellent, 7 good ,and 3 fair according to Gartland -Werley functional assessment system. Conclusions Volar LCP fixation combined with Kapandji technique is a safe and effective treatment for the unstable distal radial fractures, which could prevent reduction lost, reduce the need for bone grafting, provide with early wrist motion and avoid tendon irritation.
